Define Peritonitis
inflammation of peritoneum (membrane over GI tract)
Etiology (5)
D/T:
-bacteria (especially E. coli)
-chemical irritation (i.e. bile d/t hernia or esophagitis)
-perforated ulcer
-ruptured appendix
-PID (pelvic inflammatory disease)
Pathology (2)
thick, sticky exudate forms (d/t bacterial infection)
-seals off perforation stopping further exit & spreading if GI contents
-contains & restricts bacteria, localizing & limiting the inflammation

SNS stops peristalsis (limiting further exit of GI contents into body cavity)
Why can the irritant spread rapidly and easily systemically?
peritoneum is large and richly vascularized
Define Ileus
cessation of peristalsis in the gut
Manifestations (3)
-pain

SEVERE SYSTEMIC MNFTS
-Vasodilation: blood is shunted from other areas to perfuse inflamed area; altered perfusion; inadequate O2
-Fluid Shift: ileus; content of gut retained; fluid & air accumulate in GI tract; inc. pressure; inc. fluid secretion (i.e. mucous), compounding the retained gut contents
Treatment
-decrease inflammation
-IV antibiotics (if bacterial infection)
-surgery
-fluid & electrolytes
-pain meds (narcotics - d/t severity of pain)
