Define Peritonitis
inflammation of peritoneum (membrane over GI tract)
Etiology (5)
D/T:
-bacteria (especially E. coli) -chemical irritation (i.e. bile d/t hernia or esophagitis) -perforated ulcer -ruptured appendix -PID (pelvic inflammatory disease) |
Pathology (2)
thick, sticky exudate forms (d/t bacterial infection)
-seals off perforation stopping further exit & spreading if GI contents -contains & restricts bacteria, localizing & limiting the inflammation SNS stops peristalsis (limiting further exit of GI contents into body cavity) |
Why can the irritant spread rapidly and easily systemically?
peritoneum is large and richly vascularized
Define Ileus
cessation of peristalsis in the gut
Manifestations (3)
-pain
SEVERE SYSTEMIC MNFTS -Vasodilation: blood is shunted from other areas to perfuse inflamed area; altered perfusion; inadequate O2 -Fluid Shift: ileus; content of gut retained; fluid & air accumulate in GI tract; inc. pressure; inc. fluid secretion (i.e. mucous), compounding the retained gut contents |
Treatment
-decrease inflammation
-IV antibiotics (if bacterial infection) -surgery -fluid & electrolytes -pain meds (narcotics - d/t severity of pain) |
