Hypopituitarism
Disease of pituitary gland itself or of the hypothalamus
Complication of radiation therapy to head and neck area
Result: Extreme weight loss, emaciation, atrophy of all endocrine glands/organs, hair loss, impotence, amenorrhea, hypometabolism, hypoglycemia
Coma and death occur if missing hormones are not replaced
Hypophysectomy
Treat pituitary gland tumors
Cushing's syndrome (excessive ACTH)
Menstruation ceases/infertility occurs
Replacement therapy with corticosteroids and thyroid hormones is necessary
Diabetes Insipidus - Overview
Disorder of posterior lobe of pituitary gland
ADH deficency
Polydipsia
Large volumes of dilute urine
May occur with meningitis, encephalitis, TB
Failure of renal tubules to respond to ADH - may be d/t hypokalemia, hypercalcemia and meds such as Lithium and Declomycin
Diabetes Insipidus - Clinical Manifestations
Specific Gravity - 1.001-1.005
No abnormal substances
Drinks 2-20 L of fluid/day
Fluid Deprivation test for Diabetes Insipidus
Withhold fluids 8-12 hrs.
Weigh frequently during the test
Unable to increase specific gravity and osmolality of urine
Terminate if tachycardia, excessive weight loss, or hypotension develops
Diabetes Insipidus - Medical Management
Replace ADH
Ensure adequate fluid replacement
Identify underlying cause
