Source of information for nursing practice
nonresearch evidence -QI and risk management data, international, nat. and local standards, infection control data, benchmarking, retrospective, or concurrent chart reviews and clinician's expertise.
5 Steps of EBP
1. Ask a clinical question. 2. Collect the most relevant and best evidence 3. Critically appraise the evidence you gather 4. integrate all evidence w/ one's clinical expertise and client preferences and values in making a practice decision or change 5. Evaluate the practice decision or change
Problem -focused trigger
knowledge-focused trigger
(these come in question form)
___ one you face while caring for a client or a trend you see on a nursing unit. (increased # of client falls, incidence of UTI)
____ is a question regarding new info available on a topic ("what is the current evidence to improve pain management in clients w/ migraine headaches?") - Agency for Healthcare Research and Quality, the American Pain Society, AACN
PICO question
The more sp. you ask, the easier it will become to search for evidence in the scientific literature. Allows you to ask questions that are intervention focused. Or "I" can be "interest area"
PICO question Part 2
P=Patient population of interest
I=Intervention of interest
C= Comparison of interest (current standard)
O=Outcome (what result do you wish to achieve or observe as a result of an intervention)
Clinical Guidelines
The National Guidelines Clearinghouse (NGC) is a database supported by the Agency for Healthcare Research and Quality (AHRQ). It contains ___. ___ systematically developed statements about a plan of care for a specific set of clinical circumstances involving a sp. client population

Hierarchy of Evidence
Randomized Controlled Trial
Highest level of experimental research, when researchers test an intervention against the usual standard of care. Researchers assign subjects to either a control or treatment group through random assignment. When RCT is completed, the researcher will know if the intervention leads to better outcomes than the standard care
Controlled trials W/OUT (non) randomization
are not as conclusive because there is a bias -findings are distorted because of how the study was designed.
Case control studies
are often done retrospectively. ___ determines if there is an association btwn one or more predictor variables and the condition.
RCT's
gold standard for research, establish cause and effect, and are excellent for testing drug therapies or medical treatments, but it is not always the best for testing nursing interventions
Qualitative research
offers answers when trying to understand client's exp. w/ health problems & the contexts in wh/ the experiences occur. Typically quality and risk management data will not inform you about the nature or severity of problems occurring w/in the health care setting.
Nursing Research
is a way to identify new knowledge, improve professional education and practice, and use resources effectively. National Inst. of Nursing Research and Int. Council of Nurses regularly update the broad priorities for nursing research. Also, ANA, AACN, Oncology Nursing Society (ONS), and Sigma Theta Tau Int support advance in research.
Outcomes research
is research designed to asses and document the effectiveness of health care services and interventions. Response of health care industry to increased demand from policy makers, insurers, and public to justify care practices and systems in terms of improved client outcomes and costs.
Care delivery outcomes
observable or measurable effects of some intervention or action. Focus is on recipient of service
Scientific Method
Foundation of research and the most reliable and objective of all methods of gaining knowledge. Objective
Evaluation research
determines success or lack of success of a program. identifies problems w/ programs and opportunities for change.
Qualitative Research - Descriptors and feelings
Qualitative nursing research
___is the study of phenomena that are difficult to quantify or categorize. Involves inductive reasoning to develop generalizations or theories fr. spec. observations or interviews.
