SLE & Scleroderma

Chronic inflammatory autoimmune disorder of connective tissue

SLE - Skin

Discoid

Butterfly/coin-like rash

oral/nasal lesions

Scleroderma - Skin

Raynaud's phenomenon

Thick, hard

Taut, smooth, shiny

Loss of elasticity/movement

SLE - Systemic Effect

Multiple organ involvement

Arthritic changes

Pericarditis, pleural effusion, renal disease

Scleroderma - Systemic Effect

CREST Syndrome

Calcinosis - calcium deposits in the tissues

Raynaud's phenomonon - spasm of vessels in response to cold/stress; causes skin to become white

Esophageal dysmotility - acid reflux and descrease in mobility of esophagus; causes difficulty swallowing and esophageal dilation

Sclerodactyly - thickening/tightening of skin on hands and fingers casuing edema and pain

Telangiestasia - capillary dilation that causes spider-like hemangiomas mainly on plams and face

Calcinosis

calcium deposits in the tissues

Raynaud's phenomonon

spasm of vessels in response to cold/stress; causes skin to become white

Esophageal dysmotility

acid reflux and descrease in mobility of esophagus; causes difficulty swallowing and esophageal dilation

Sclerodactyly

thickening/tightening of skin on hands and fingers casuing edema and pain

Telangiestasia

capillary dilation that causes spider-like hemangiomas mainly on plams and face

SLE - Patho

Autoantibodies produced against the blood cells and/or organs; results in disturbed immune regulation and deposits of damaging immune complexes in the tissues

Sclerodrema - Patho

Lymphokines stimulateprocollagen; results in insoluable collagen deposits

Inflammatory response causes edema

Fibrotic changes in skin and eventually organs

SLE - Diagnostics

(+) ANA

Pancytopenia

(+) CRP & ESR

Scleroderma - Diagnostics

Skin Bx

(+) ANA

(+) ESR

SLE - Tx

NSAIDS, steroids

Antirheumatoid

Antimalarial - Plaquenil (causes visual changes - see meds cards)

Cytotoxins/antineoplastics - methotrexate

Plasmapheresis

Scleroderma - Tx

NSAIDS, steroids

Antirheumatoid - penicilliamine (Cuprimine)

Antiinfective - minocycline (Minocin); thought to decrease collagen thickening

SLE - Nursing Care

Assess for worsening of disease/SE of meds

Assess for kidney failure - check labs, changes in urine, may need low protein diet

Education - meds, signs of infection

Emotional support

Scleroderma - Nursing Care

Assess for worsening of disease/SE of meds

Protect digits - keep warm/wear gloves

Education - smoking cessation, exercise, meds

Emotional support
