When would a thyroid scan be ordered?
to evaluate nodules
radioactive isotopes are given orally or intravenously. benign nodules appear as warm spots b/c they take up the radionuclide malignant nodules appear as cold spots b/c they do not take up the radionuclide |
What is a nursing responsibility of a PTH study?
fasting specimen
keep sample on ice |
Describe the water deprivation test function.
Used to differentiate causes of polyuria
(Diabetes insipidus and SIADH) |
What do you administer to the patient either IV or SQ during the water deprivation test?
ADH or vasopresin
In normal people urine osmolality is normal after ADH secretion. In patients with DI, there is no or minimal response....(so their urine stays undiluted) |
What are your responsibilities as a nurse in regard to the water deprivation test?
Watch for severe dehydration
Fluid restriction either 6 hours before or right before. Test lasts 6 hours, starting at 6am. Obtain baseline weight and urine and plasma osmolality prior to test start. Discontinue test and rehydrate if patient's weight drops more than 2kg at any time. Rehydrate with oral fluids. Check orthostatic BP and pulse after rehydration. **this is on p 1248 and is confusing, but she said to know it..... |
Your patient is going to have an oral glucose tolerance test in three days. What do you advise her when she asks if she has to prepare in any way?
Refrain from smoking and caffeine starting now...avoid stress (but you just asked her to cut out coffee and smokes, so ????), and fast 12 hours before the test.
Make sure her diet (starting now) includes 150-300 g of carbs with 1500 calories a day. |
How long do you have to fast for a FASTING BLOOD GLUCOSE?
4-8 hours
What is stereognosis?
recognizing an object
place a key in their hand and have them close their eyes and tell you what it is |
What is Trousseau's sign? How do you perform it? What does it test for?
A test for hypocalcemic tetany
Inflate BP cuff above AC space greater than systolic blood pressure for 2-5 minutes. If patient has decreased calcium levels then the hands and fingers will spasm...carpal spasm |
What is Chvostek's sign? how do you perform it? What does it test for?
A test for hypocalcemia. If they have low calcium, then when you tap on the angle of their jaw, the facial muscle on the same side of the face will twitch (sometimes around the lips and nose too) and cause a grimace due to repeated contraction of the facial muscle. Testing for LOW calcium.
What conditions in this unit might you suspect hypocalcemia?
hypoparathyroid
post op thyroidectomy (b/c they accidentially took out the parathyroid glands) |
