1. Perimenopausal women b/c they may not be able to use hormonal methods such as bcp b/c of hx of HBP, thromboembolic disease, or cigarette smoking
2. Postpartal women: they can breastfeed (lacation Amenorrhea method)
involves detecting when a woman is fertile so she can use periods of abstinence during that time.
Fertility Awareness (Natural)
*Temp: at time of ovulation rises a full degree

CHEMCIAL/HORMONE METHOD
Action of BCP aka OC's or COC's (combination of oral contraceptives
composed of synthetic estrogen combined w/ synthetic progesterone (progestin).
estrogen acts to suppress FSH & LH, suppressing ovulation.
-progesterone action complements that of estrogen by causing a decrease in the permeability of cervical mucus, thereby limiting sperm motility and access to ova.
-Progesterone interferes with tubal transport and endometrial proliferation to such degrees that the possibility of implantation is significantly decreased.
What contraceptives are good for women w/ liver disease?
-COC's -vaginal ring
b/c hormones released are absorbed directly by the mucous membrane of the vagina, thereby avoiding a “first pass” through the liver,
What are the Advantages of DEPOPROVERA?
- used during breastfeeding.
-reduction in ectopic pregnancy, endometrial cancer, endometriosis, and,
-reduction in the frequency of sickle cell crises
-doesn't interfere w/ sex
Natural contraceptives are contraindicated in who?
Adolescent b/c anovulatory menstrual cycles for several years after menarhe
-require them to say "no" on fertile days (difficult to do when under pressure)
What are the Contraindications to oral CONTRACEPTIVE oral?
-breastfeeding
- > 35 y/o
-CV disease risk factors: older age, Smoking, Diabetes,
Hypertension
-DVT -Heart disease -stroke
-migraines
-liver (tumors or cirrhosis)
Barrier Method:
blocks the action b/w the cerix andadvancin spersm so the sperm cannot enter the uterus of Fallopian tubes
Ie. of barrier methods
condoms (mechanical
diaphragm (mechanical)
sponge (mechanical)
cerivcal cap (Mechanical)
-Spermicide (chemical)
What are the Disadvantages of Barrier methods?
-sexual enjoyment maybe lessened
Where does Estrogen and Progesterone come from?
anterior piturary gland
-estrogen & progesterone stops ovulation
What is th mechanism of action for the Hormone method (oral, transdermal, vaginally, by inplantation or injection)
thickens mucus and urteral wall
-kills sperm
Surgical Methods:
-vasectomy: vas deferen cut & tied
-tubal ligation: fallopian tubes are occluded preventing passage of sperm and ova
IUDs are contraindicated for what type for client?
-multiple sex partners (STDs)
-Never been pregnant (Adolescent)
-uterus distorted in shape
-anemia -Heart disease
-severe dysmenorrhea
-hx of ectopic
IUDs are best for what clients?
Paragard ( effective be used up to 10 years, hormone free)
-Mirena: prevents sperm from reachin egg & thickens cervical mucus, ues progesterone. (effective up to 5 years)
-perimenopausal

-postpartal (ok to use)
Depo Provera:
(hormonal method)
-need increase Ca+
-contains only progesterone
-safe to use while BREASTfeeding(b/c it only contains progesterone)
-stops ovaries from releasing eggs & thickens cervix
Depo Provera is not routinely prescribed to _________.
adolescents to protect their bone mineral density
What is the mechanism for Implantation devices?
(Implanion)
Can be safely used in Adolescents
-can be used during Breastfeeding
-slowly release the hormone (Progestin), suppressing ovulation, stimulating thick cervical mucus
-stops ovaries from releasing te egg
Barrier Methods are contraindicated in what type client?
-Adolescent
-perimenopausal women
stops ovaries from releasing egg and keeps sperm from reaching egg
-has highest doses of estrogen & proges
Morning after pill (give 1st pill w/in 72 hous)

estrogen & progesterone prevents th release fo FSH
Women who are taking COC's should notify their MD if what complications occur?
myocardial or thromboembolic:
-chest pain
-SOB
-Severe HA, Leg pain
-Eye problems Blurred vision
What method is this?
-Adolescent should have their menstrual cycles for at least 2 years before beginning ______.
COC's (combination oral contraceptives)
-b/c estrogen has the side effect of causing the epiphyses of long bones to halt growth
