then.....
Open the airway manually..
Elevate the toungue and insert a simple adjunct (oral or naso).....
Examiner says "There is no gag reflex present and the patient accepts the adjunct."....
Ventilate the patient immediately with bag-valve-mask device unattached to oxygen with 2 rescue breaths
Ventilates patient with room air....
Examiner now informs canidate that "Ventilation is being performed without difficulty and pulse oximetry indicates the patient's blood oxygen saturation is 85 %......
Attach oxygen reservoir to bag valve mask device and CONNECT TO HIGH FLOW OXYGEN REGULATOR AT 12-15 LPM.......
Ventilate the patient at a rate of 10-12 per minute (once every 5-6 seconds) with appropriate volumes.....
After 30 seconds the examiner auscultates and reports breath sounds are present and equal bilaterally and medical direction has ordered insertion of a supraglottic airwa. The examiner now takes over ventilation.......
Direct assistant to PRE OXYGENATE PATIENT.....
Check and prepare supraglotic airway device (ie ballons, syringes setup ect)....
Lubricates the distal tip of the device (may be berbalized).....
Examiner removes teh OPA and moves out of the way when the canidate is prepared to insert the device.......
Positions the head properly...
Performs a tongue-jaw lift...
Inserts device to proper depth....
Secures device in patient (inflates cuffs with proper volumes and IMMEDIATELY REMOVES THE SYRINGE....
SECURES THE STRAP.....
Ventilates patient and confirms proper ventilation (correct lumen and proper insertion depth) by AUSCULTATING BILATERALLY OVER LUNGS AND OVER THE EPIGASTRUM.....
Adjust ventilation as necassary(Ventilates through additional lumen or slightly withdraws tube until ventilation is optimized)....
Verifies proper tube placement by secondary confirmation such as....
1.Capnography, capnometry, colormetric device or EDD.
2.Visulize passing the vocal cords
3.Condensation in the tube
4.Equal breath sounds with rise and fall of both sides of the chest.....
The examiner must now ask the canidate "How would you know you are delivering appropriate volumes with each ventilation?".....
Secure the device and confirm that the device remains properly secured....
Ventilates the patient at proper rate and volume while observing capnography/capnometery and pulse oximetr
Ventilations continue and state that you are complete
