Glucose tolerance test

Referred to as GTT.


There are 3 types of glucose tolerance test



●2 hours post prandial mean during or relating to dinner or lunch



●Usually used to determine diabetes in pregnant females



●concern regarding weight gain in mother and usually deliver adnormally large babies



3 hour glucose tolerance

Used to diagnose diabetes mellitus (hyperglycemia )



Ex: of symptoms include frequent urination and increase thirst

5 hour glucose tolerance

Used to diaginse hypoglycemia (low blood sugar)



□some example of symptoms include anixtey, sweating, palpitations and nausea

Produce of 2 hour Post Prandiat (2 hours PP)

1- ask patient if they are NPO



2-do not proceed if they have eaten



3-draw fasting blood sugar using gray top tube



4- secure fasting urine specimen



5- administer to adult patient 100 grams of glucose (glucola, koladex,ext)



6- start stopwatch or note time



2 hours after ingestion of glucose, draw second blood sugar and secure urine sample

Interpretation

Both urine should be negative for sugar



● the FBS should be 70 to 100 mg/dl



●second blood sugar should be b/t 120 to 140 mg/dl


(Sugar about 140 mg/dl, suspect diabetes)



☆futher diagnostic work up may be required

Procedure for 3 hour Glucose Tolerance

1- ask patient if NPO



2- do not procede if they eaten



3- explain to patient that they are not eat, smoke, or exercise during test



4- they are to remain in area for duration of test (3 hours)



5- encourage them to drink water

Procedure for 3 hour glucose test

6- have supplies ready, (gray top or red top)


Oral glucose dose, urine containers, emesis basin in case they become nauseous, gauze pads, alcohol, etc..



7- draw FBS and secure urine samples, some facilities check unrine sample for sugar and nofoty physician if positive


●administer glucose load

Procedure for 3 hour glucose tolerance

■advise patient that all must be consumed within 5 minutes. Note time



■if patient become nauseous give them ice to suck on or have them lay down



■if they vomit during within 30 minutes of administration, estimate how much fluid was regurgitated and call doctor.


(Physician may cancel test )



■ draw blood sugar and secure urine sample at 1/2, 1, 2 and 3 hours after administration of glucose load.



■ make sure blood and urine samples are labeled within time sample was obtained

5 hours gluocse tolerance (test)

GTT- pancreas does not produce enough insulin



●can not tolerate sugar



● 100 grams glucose post prandial

Blood culture

■ produce is used to diagnose septicemia



■septicemia is presence of baterica in blood stream can be fatal if untreated



■ if multiple test are requested on patient, blood culture are drawn first



■use speical types of evacuated bottles that comtain food for baterica to use during growth (media)

Aerobic- is for baterica that requires air for growth



Anaerobic - for baterica that do not require air for growth

Important items to remember are that tops of vacutainer and puncture site must be sterilized w/ an iodine compound and that collected bottles must be labeled with patient identification information , date and time specimen was drawn

Bleeding time

A test which measure length of time required for a superficial subcutaneous cut to stop bleeding.



Test is used to detect platelet function disorder
