What is type of information does the spinothalamic tract carry?
pain and temperature
Trace the path of the ALS tract from dorsal root to the ALS.
primary neuron- cell bodies in the dorsal root ganglia, enter dorsal horn and can ascend or descend in Lissauer's tract. Synapse in dorsal horn and cross thru ant. white commissure to the ALS
Where does the secondary nucleus of the ALS terminate?
Ventral Posterior Lateral nucleus of thalamus
Trace the route of the ALS to from the thalamus to the brain.
Cell bodies of tertiary neuron in the VPL of the thalamus, axons ascend via the posterior limb of the internal capsule to the postcentral gyrus and the paracentral lobule.
Where does the painful stimuli of C-fibers ascend to the brain?
The multisynaptic spino-reticulo-thalamic pathway going through the reticular formation and into the midline nuclei and interlaminar nuclei before passing to the cortex.
What are the areas of the cortex that the spino-reticulo-thalamic go to?
orbitofrontal cortex, insula, and anterior cingulate gyrus
What is the most medial portion of the dorsal column?
the sacral region
In the ALS and DCML, collateral branches are involved in what?
Local reflexes (withdrawal, etc)
What is the organization of the dorsal column and what are each of the locations associated with?
Gracile tract (medial)- T7-S5 so lower trunk and legs. Cuneate tract (lateral)-c1-T6 upper trunk and arm
What sensory information is relayed by the DCML tract?
Sensory to recognize objects, interpret drawings on skin, proprioception in dark, direction of movment of objects across the skin
Where do the axons of the DCML system cross the spinal cord?
Cross the midline anterior to dorsal column nuclei in the caudal most medulla
In the medial laminiscus tract, where are the leg tracts located in the pons?
laterally on the ML
Where do axons of the ML terminate?
VPL thalamus
What are the three branches of the trigeminal ganglion? What areas of the head do they not innervate?
Ophthalmic (V-1), Maxillary (V-2), Mandibular (V-3). It does not innervate the back of the head nor the angle of the jaw
What are the two main nuclei of the trigeminal nerve? What do they control?
Principal tri. nucleus- fine touch sensory, joint position, vibration. Spinal trigeminal nucleus- pain and temperature
What does the spinal trigeminal nucleus innervate? What is it's tract?
Pain and temperature of face. It enters thru the trigeminal ganglion then dips down into the spinal cord
What is the spinal tract continuous with?
dorsolateral fasciculus (Lissauer's tract)
Where do the neurons (secondary) of the spinal and principal trigeminal nuclei go?
Ventral Posterior M, then synapse and tertiary neurons to go the postcentral gyri
What sensory information will be affected if there is a lesion of the postcentral gyrus?
diminished ability to distinguish size, texture, shape, and position. Pts complain of tingling, heaviness, like wearing off Novocain.
What areas of the brain are involved in pain?
anterior cingulate gyrus, orbitofrontal cortex, and insular cortex
What part of the brain is important for recognizing objects by palpation?
posterior superior parietal cortex 5,7
If a patient has a lesion in the posterior superior parietal cortex, what might they have wrong?
Astereogenesis, agraphesthesia, intentional mvmnts, cortical neglect (if to non-dominant side)
Sensory dissociation only occurs where?
below the medullary-pontine junxn, above that the ALS and DCML are too close together
Where is the gracile and cuneate nuclei respectively?
Both in the medulla. Cuneate is more lateral. THey are in the dorsal column
