Nephrotic syndrome: overview, causes
|
Charaterized by massive proteinuria (>3gm/24hrs), hypoalbuminemia, severe edema, hyperlipidemia, lipiduria and (in adults) increased risk of venous thromboembolism.
Causes: Minimal change disease (most common in children/causes foot process effacement), Focal Segmental Glomerular sclerosis (African Americans adults), membranous nephropathy (White adults), systemic causes (diabetes and amyloidosis) |

Tx for:
MCD- Responds rapidly to steroids FSGS: Associated w/HIV and heroin use, frequently accompanied by nephretic complications. Responds poorly to steroids. Membranous nephropathy: associated w/hep. B, lupus and various carcinomas |

Characterized by modest proteinurea (<3gm/24hrs), hematuria, renal insufficiency, htn and urinary dysmorphic rbcs and rbc casts.

Poststreptoccal glomerulonephritis
|
An acute nephritic syndrome.
Occurs 1-2 wks after streptococcal sore throat or skin/soft tissue infection. ASO or anti-DNase titers can be used to document prior infections. Typically resolves w/o specific therapy Most often seen in children Severe edema is common |

Crescent of rapidly progressing glomerulonephritis
|
Glomerulonephritis w/rapid onset and poor prognosis.
Rapid progression to renal failure. Number of crescents=indicates prognosis 3 types (distinguished by immunoflourescence): 1. pauci-immune (antineutrophil cytoplasmic antibody-associated)disease. Wegner's granulomatosis/microscopic polyangitis) 2. Antiglomerular basement membrane syndromes (Goodpasture's syndrome) 3. End-stage immune-complex mediated diseases. |

Immunoglobulin A nephropathy and Alports syndrome
|
Frequently present w/asymptomatic microscopic hematuria.
IgA nephropathy: most common primary glomerular disease world wide. Alport's syndrome: caused by inherited defects in collagen IV and often accompanied by hearing loss. Split basement membrane. Nerve deafness and ocular disorders |
