A. AR 623-205.
Q. Who receives an NCOER?
A. E-5's and above in the Army, Army National Guard, and the United States Army Reserve.
Q. There are seven different types of NCOER's. What are they and give a brief explanation of each.
A. Initial - Prepared for soldiers with 3 rated months after promotion to E-5; Annual - Prepared 12 months after the last issued NCOER; Change of Rater - Prepared when the soldier has a change of rater, provided there are at least 3 rated months involved; Special Reports - For circumstances so outstanding or poor as long as there are 30 days accounted for and the report is approved by a field grade officer; Relief for Cause - For soldier whose performance warrants this NCOER and has a minimum of 30 days. 30 day requirement can be waived under certain circumstances; Directed Reports - Sent down by DA; Complete the Record - Sent down by DA.

A. A listing of personnel eligible to receive an NCOER. It should be posted in the company or section in a place visible and readily available for everyone.
Q. What is an NCOER?
A. A written report providing the individual soldier formal recognition for performance and personal traits and a basis for counseling by rating officials or career development.
Q. What are some of the uses of an NCOER?
A. Used as support for personnel actions like promotions, school selection, assignment, MOS classification, CMF designation and qualitative management.
Q. What forms are used for the NCOER?
A. DA 2166-7-1, NCO Counseling Checklist/Record; DA 2166-7, NCO Evaluation Report.
