Mechanism of action
|
blocks opiate receptors in brain, reversing the respiratory depression, pinpoint pupils, analgesia and sedation caused by narcotics.

Indications
|
suspected narcotic overdose in patients with respirations </= 12/min and signs of narcotic use (drug paraphernalia, pinpoint pupils)

dosage form

0.4mg or 0.5 mg/ml, 1ml ampule or 10ml vial
0.8mg, 1mg or 2 mg / 2ml, prefilled syringe |

Adult dose
|
IM:
0.8, 1, or 2 mg IM - may repeat once IVP: 0.4-1 mg IVP titrated to an adequate respiratory rate/tidal volume and a responsive patient w/out signs of withdrawal. Repeat dose every 2-3 minutes as needed. ET: 4mg once |

Pediatric dose
|
0.1 mg/kg IVP titrated to age appropriate respiratory rate. Repeat dose every 2-3 minutes as needed to a maximum of 1 mg/dose.
0.1 mg/kg IM once to a maximum of 1 mg, may repeat once. |

Precautions/comments
|
since duration of narcotic action may exceed that of naloxone, watch for recurrent respiratory depression and coma.
Can precipitate withdrawal syndrome and combative behavior in addicts |
