Orig: Tip of coracoid process

Ins: Middle of medial border of humerus
Action: Flexes and adducts shoulder joint

N: Musculocutaneous n. (C5,6,7)
Biceps brachii
Orig: LONG HEAD from supraglendoid tubercle of scapula
SHORT HEAD from tip of coracoid process

Ins: Heads insert together on radial tuberosity, along with bicipital aponeurosis
Action: Flexes shoulder joint (long head may assist with sbduction if the humerus is laterally rotated)

N: Musculocutaneous n. (C5,6)
Brachialis
Orig: Lower half of front of humerus

Ins: Ulnar tuberosity, coronoid process of ulna
Action: Flexes elbow

N: Musculocutaneous n. (C5,6) and radial n. (C7) to small lateral part
Triceps brachii
Orig: LONG HEAD from infraglenoid tubercle of scapula
LATERAL HEAD from posterior and lateral surfaces of humerus above radial groove
MEDIAL HEAD from posterior surface of humerus below radial groove

Ins: All three heads insert together on upper surface of olecranon process of ulna and deep fascia of forearm. A few fibers insert into fibrous capsule of elbow joint
Action: Extends elbow, Long head aids in extension and adduction of arm

N: Radial n. (C6,7,8)
Anconeus
Orig: Posterior surface of lateral epicondyle of humerus

Ins: Lateral side of olecranon and upper fourth of posterior surface of shaft of ulna
Action: assists triceps in extending elbow joint

N: Radial n. (C7,8)
Trapezius
Orig: UPPER FIBERS from external occipital protuberance, medial 1/3 of superior nuchal line, ligamentum nuche, spine of C7
MIDDLE FIBERS from spines of T1-T5
LOWER FIBERS from spines of T6-T12

Ins: UPPER FIBERS into lateral third of clavicle
MIDDLE FIBERS into acromion and crest of spine of scapula
LOWER FIBERS into an aponeurosis at medial end of spine of scapula
Action: UPPER FIBERS elevate, retract and rotate scapula upward. With insertion fixed, they extend, side bend to same side and rotate head to opp. side
MIDDLE FIBERS retract scapula
LOWER FIBERS retract, depress and rotate scapula upward

N: Spinal accessory n. (XI cranial n.) and ventral rami of C2,3,4
Latissimus dorsi
Orig: Spines of T7-T12, thoracolumbar fascia from lumbar and sacral spines and posterior third of iliac crest. Lower 3 or 4 ribs. A slip from inferior angle of scapula

Ins: Intertubercular groove of humerus (anterior)
Action: extends, adducts and medially rotates humerus. With humerus stable, contraction of latissimus can raise the pelvis toward upper body (depressing shoulder girdle). Acting bilaterally, tilts pelvis anteriorly. Acting unilaterally, assists with lateral flexion on the trunk

N: Thoracodorsal n. (C6,7,8) from posterior cord of brachial plexus
Levator scapulae
Orig: Transverse processes of first 4 cervical vertebrae

Ins: Medial border of scapula from superior angle to base of spine
Action: Elevates and rotates scapula downwards. With insertion fixed, it extends, side bends and rotates neck to same side

N: Dorsal scapular n. (C5), ventral rami of C3,4
Rhomboideus major
Orig: Spines of T2-T5

Ins: Medial border of scapula from base of spine to inferior angle
Action: Retract, elevate and rotate scapula downwards

N: Dorsal scapular n. (C4,5)
Romboideus minor
Orig: Lower part of ligamentum nuchae, spines of C7 and T1

Ins: Medial border at base of spine
Action: Retract, elevate and rotate scapula downwards

N: Dorsal scapular n. (C4,5)
Pectoralis major
Orig: clavicular head from anterior surface of sternal half of clavicle
Sternocostal head from anterior surface of sternum and costal cartilages of first six or seven ribs

Ins: Crest of greater tubercle of humerus
Action: Both heads ADDUCT and MEDIALLY ROTATE humerus
Clavicular head FLEXES, HORIZONTALLY ADDUCTS, MEDIALLY ROTATES humerus
Sternocostal head DEPRESSES shoulder girdle and obliquely ADDUCTS humerus toward opposite iliac crest

N: Medial and lateral pectoral nerves (C5-T1) from medial and lateral cords
Pectoralis minor
Orig: Upper margins and outer surfaces of ribs 2 or 3-5 near their cartilages

Ins: Medial border and upper surface of coracoid process of scapula
Action: With origin fixed, ANTERIOR SCAPULAR TILT
Assists in forced inspiration

N: Medial pectoral n. with fibers from lateral pectoral n. (C6,7,8,T1)
Subclavius
Orig: Upper surface of first rib and its costal cartilage and their junction

Ins: Lower surface of middle third of clavicle
Action: Probably pulls clavicle downwards and forwards and stabilizes it during movements of shoulder

N: Nerve to subclavius (C5,6) from upper trunk of brachial plexus
Serratus anterior
Orig: Digitations from outer surfaces and upper borders of upper 8 or 9 ribs on anterolateral thoracic wall

Ins: Entire length of costal surface of medial border of scapula from superior to inferior angles
Action: Principal protractor of scapula
Chief muscle in all reaching and pushing movements
Lower fibers rotate scapula upwards (may depress)

N: Long thoracic n. (C5,6,7) from roots of brachial plexus
Deltoid
Orig: Anterior fibers from lateral third of clavicle (anterior border and superior surface)
Middle fibers from acromion (lateral margin and superior surface)
Posterior fibers from lower edge of crest of spine of scapula

Ins: Deltoid tuberosity of humerus on anterolateral surface just above its midshaft
Action: Middle fibers are principal ABDUCTOR of humerus
Anterior fibers FLEX and MEDIALLY ROTATE humerus
Posterior fibers EXTEND and LATERALLY ROTATE humerus

N: Axillary N. (C5-6) from posterior cord
Supraspinatus
Orig: Medial 2/3 of supraspinous fossa of scapula

Ins: Superior face on greater tubercle of humerus
Action: Assists deltoid in abduction of humerus

N: Suprascapular n. (C4-6) from upper trunk
Infraspinatus
Orig: medial 2/3 of infraspinous fossa of scapula

Ins: middle facet on greater tubercle of humerus
Action: Principal lateral rotator of humerus
N: Suprascapular n. (C5-6)
Teres minor
Orig: upper 2/3 of dorsal surface of lateral border of scapula

Ins: Lowest facet on greater tubercle of humerus
Action: LATERAL ROTATION of humerus

N: Upper and lower subscapular nerves (C5-6)

Orig: medial 2/3 of subscapular fossa on costal surface of scapula

Ins: Lesser tubercle of humerus
Action: Principal MEDIAL ROTATOR of humerus
