Reflex Sympathetic Dystrophy
Reflex Sympathetic Dystrophy (RSD) is a d/o of extremities characterized by:
-pain
-swelling
-limited ROM
-vasomotor instability
-skin changes
-patch bone demineralization

-begins after injury, surgery, or vascular event like MI or stroke
RSD: stages
(complex regional pain syndrome: Stages 2 and 3)

Stage 1:
*pitting edema of hand,
non-itting edema of fingers
1. burning, throbbing pain
2. diffuse aching
3. sensitive to hot/cold (d/t vasomotor disturbances)
4. localized edema

Stage 2:
-thickening of skin and non-pitting edema

Stage 3:
-contractures of digits
-waxy trophic skin changes
-brittle nails
claw limb
RSD
Tx complex regional pain syndrome
early intervention essential

Stage 1:
TCA + topical cream
alpha blocker
PHYSICAL THERAPY IS KEY

If no response: stellate ganglion block + corticosteroids

If no response: sympathectomy
dupuytren's contracture
progressive fibrosis of palmar fascia==>puckering and tethering of skin

-repetitive handling tasks or vibration (jackhammers)

-exquisite local tenderness
-pain on motion and at rest
reduced active ROM, nl passive ROM
soft tissue d/o, i.e. bursitis
decr in both active and passive ROM
joint abnormalities
bursitis: tx?
-remove aggravating faactors
-ice or heat, rest, NSAIDS
-inject corticosteroids
