Syphilis (bad blood disease): Can become systemic

CA: Treponema pallidum

Treatment: Penicillin

Reservoir: Human

Transmission: direct sexual contact, congenital

Prevention Control: abstinence, safer sex, monogamy
Primary stage (3-6 wks after initial infection):
-depressed, hard, painless lesions on genitals, anus, mouth, fingers, etc.
-infectious, heals itself
-curable
-chancre, lesion

Secondary stage (4+ weeks later):
-generalized rash, can affect palms and soles
-genital/oral lesion
-fever, sore throat, headache
-infectious lesions can recur late in this stage
-highly infectious
-last chance to cure
-latent stage begins when recovered from secondary stage & can last years

Tertiary stage (3-30 years after initial infection)
-gummas (soft, rubbery lesions) form in internal & external tissues (you must have surgery to remove these)
-cardiovascular syphilis
-neurosyphilis effects central nervous system
-NOT contagious

Children may develop
-hutchinsons teeth
-saber shins

Congenital: can be passed to the newborn or the unborn.
Gonorrhea (the drip, clap & heat)
Can become systemic

CA: Neisseria gonorrhoeae

Symptoms: Male urethra inflammatory symptoms:
-pus discharge
-penis warmth/burning
-penis pain (clap to provide relief)

Women are often asymptomatic
-pus discharge mixes with vaginal secretions
If untreated, leads to:
-sterilization
-endocarditis
-meningitis
-gonococcal arthritis
-proctitis

Treatment: Penicillin

Reservoir: Humans

Transmission: direct sexual contact, gonococcal opthalmia (eye inflammation) transmitted to 1) newborns & 2) kids/adults via contaminate hands

Prevention:

abstinence & safer sex
Chancroid

CA: Haemophilus ducreyi
Symptoms:
-soft, painful, depressed lesions/chancres on external genitalia
-spreads causing necrosis
-swollen lymph nodes in groin
-can spread to breasts, lips, tongue and other mucosal and skin tissues

Treatment: ABC, sulfa drugs

Reservoir: Humans

Transmission: direct sexual contact

Prevention: abstinence & safer sex
Herpes Simplex Type II (chronic)

CA: Virus (HSV-2)
Symptoms:

-painful, raised lesions
-swollen lymph nodes
-burning, itching, painful urination
-urethral/vaginal discharge
-female: on vulva, vagina, cervix, perineum
-male: on glans, foreskin and shaft
-primary lesions 6-8 days after contact, disappear 2-3 weeks later
-recur periodically

Treatment: NS & Chemotherapy

Reservoir: Humans

Transmission: direct sexual contact, congenital

Prevention: abstinence, safer sex and monogamy.
AIDS: Acquired Immune Deficiency Syndrome (systemic):

CA: Retrovirus

*pneumocystis jiroveci (pneumocystis pneumonia), not carinii now*
Early symptomatic:
-flu like symptoms
-rashes, itching
-mouth sores

Later symptomatic:
-swollen lymph nodes
-constant fatigue
-diarrhea, weight loss
-decreased appetite
-fever, night sweats

Advanced:
-memory loss
-coordination loss
-decision making trouble

Opportunistic infections:
-Karposi's sarcoma
-pneumocystis pneumonia
-tuberculosis

Treatment: Chemotherapy

Reservoir: Human

Transmission: Vaginal/anal/oral sex, congenital, sharing needles.

Prevention control:
-abstinence
-safer sex
-monogamy
-don't share needles
-get tested if exposure was possible
