staph- adults
strep pneumonia/h influenza- kids, pink eye

**treat with topical AB if s aureus, others are self limited

**can be in any age group. seen in low SES, poor hand washing
do you get corneal scarring with inclusive conjunctivitis
nope. its caused by chlamydia trachomatis and we get mild discormort with little discharge and lots of papillae on lower conjunctival membrane

**we get problems with repeat infections
**leading cause of PREVENTABLE blindness

4. scarring, inversion of eyelashes

**common cause of preventable blindness
**repeat infection (poor sanitation, handwashing)
**constant abrasion of cornea by inverted lashes
**common to have 2 bacterial infections
**dry eye is common

erythromycin, tetracycline

WASH HANDS nad prevent reinfection
what is opthalmia neonatorum
conjunctivitis of the newborn (less than a month old)

**colonized as babe passes through birth canal
*prophylaxis against it bc it can be bad for the immature immune system (chlamydia)
also caused by goniorrhea
why do babes get silver nitrate drops
prophylaxis against conjunctivitis of the newborn caused by chlamydia or gonorrhea
what do we do when a mom has gonorrhea and passes it to her baby
conjunctivitis of the newborn, typically seen in 2-5 days

1/2 of babes will be affected

SYSTEMIC AB, topical wont be enough
whats the dif in conjunctivitis of the newborn caused by gonorrhea and chlamydia
1. gonorrhea- 2-5 days is apparent. needs systemic AB. BL

2. 5-10 days after delivery. can be eye or lung disease. UL. NO follicle formation is evident as in adult inclusice conjunctivitis

tx with erythromycin (NOT tetracycline)

reactivation (recrudesence) by immune suppression- stress, sunlight, fever

can be in the stroma or epithelium

Epithelium- see lightning bolts in the eye "dendritic figures", more common

Stroma- necrotizing, non-necrotizing
what are hte clinical manifestations of HSV Keratitis
recall it can be stroma (more serious, necrotizing or not) or epithelium (more common, lightning bolts)

**ulcers
**irritation, tearing
**no pain
**no photophobia
what does VZV recrudesence look like in the eye
varicella zooster

**complication of shingles.
**trigeminal ganlion is involved and can involve the eye

Dx:
clinical findings- corneal scrapings
HSV- epithelial lightning bolts, multinucleated giant cells

Tx:
acyclovir
treat shingles ASAP

NOT acyclovir as are other HSV tx

Ganciclovir, foscarnet. bet we still get 50% reactivation
what is the outcome of HSV/VZV for AIDS pts
prbly loose sight

**healthy ppl also prbly will loose sight but they are less likely to get it in the first place
what is Endophthalmitis? when does it happen
infection of several tissue layers of the eye (present in humor(s))

1. complictino of cataracts surgery
2. post traumatic
3. Endogenous- blood infection, metastatic infectino (candida) R eye more common. Bacillus cereus in IV users
4. uncontrollded infectinos of cornea or orbital cellulitis
what is bacillus cereus associated with
IV drug use get endogenous cause of endophthalmitis

**other causes are cataracts, injury or uncontrolled infection
