Nonconventional Antipsychotics Action
Acts on serotonin, adrenergic (catecholamine) & cholinergic receptors. Selectively blocks dopamine receptors in the limbic system instead of neostriatal area (preventing motor side-effects seen with 1st generation antipsychotics).
Nonconventional Antipsychotics S/E
Bonemarrow depression/Agranulocytosis RARE but serious. Requires weekly CBC/WBC 1st 6months then biweekly. Very sedating.
Olanzapine (Zyprexa) Action
blocks som serotonin receptors and dopamine receptors (D2). Antagonizes D1 & D4 receptors,serotonin 5-HT2C & 5HT3 receptors, alpha adrenergic & H1 receptors.
Olanzapine (Zyprexa) S/E
somnolence,psychomotor activation (agitation, nervousness, insomnia), dizziness, weight gain and dry mouth.
Olanzapine (Zyprexa) NSG concerns
Smoking may increase clearance by 40%. Long half-life—may be given 1 X/day—better compliance.
Quetiapine (Seroquel) ----XR form also Action

Headache, somnolence, dizziness,constipation, postural hypotension. Least likely to cause EPS.

Need yearly vision exam with regular use. XR do not crush, give on empty stomach.

Must give with food or only 50% absorbed.
