Maslow’s hierarchy in mental health

When assessing someone that presents with mental health issues… always think about Maslow’s hierarchy because it is how it will present itself.


Physiological-


Safety-will be established when the person will be able to think rationally. Secure food, shelter, cause harm to self and others


Love & belonging-client is in then center and it’s everything that interacts with the client. Work, school, spouse, kids… when a patient can cope with stress and foster relationships, they are known to be mentally healthy


Self-esteem-be able to view themselves in a positive way. Ask how do you view yourself?


Self actualization- the person has reached their purpose… do you know why you are here? Purpose to live? Most have not reached this, but most are reaching it. People grow emotionally and psychosocially it is a motivation to continue to grow.


So when there is dysfunction in one or more of these we can describe it in terms of a mental health crisis.


The most important is physiological issues, the class is going to focus of the upper 4…


Example if a client has self esteem and self actualization problems, which one do you Focus on first?? The self esteem… always work from the bottom up. Follow the hierarchy.


When does it become a disorder?? Remember a disorder is a collection of symptoms.


When symptoms are shared within different people. From different cultures and different ethnicities, these symptoms are occurring across the human lifespan… this is called a trend and we can simply say schizophrenia, major depressive disorder then you’ll know what we are talking about.



What does it mean to be mentally healthy??

3. Foster social relationships


4. Goal orientation

How does a person seek help?

Voluntary admission-


Involuntary hold- does one thing, “REVOKES RIGHT TO FREEDOM” lasts for 72hrs—but maintains the right to


1. Refuse treatment-special and the rest of the rights can be revoked if it causes danger to the clients


2. Right to privacy- this can be preached to a third party that is not apart of the healthcare team… when abuse is suspected, and report it. Danger to others, mandated by law to contact local police and inform them the client wants to kill someone after his release. And danger to self. Example will be if you were working for a suicide hotline and a client verbalizes that they want to kill themselves. Call police department. And court ordered and must divulge information about the client under oath.


3. Right to dignity


4. Right to visitors/make calls/shelter/food


What is the criteria to place a client in an involuntary hold?? One of 3 criteria must be met…


1. A danger to themselves


2. A danger to others


3. Gravely disabled (symptoms are inhibiting them from completing their ADLs)


When the person continues to refuse any medical intervention the following occurs?


The client can be taken to mental health court.


The client may say…


1. I want fight for my right for my freedom back


2. The facility wants to revoke right to refuse- that way the client will be given the intervention ex. Place an NG tube for medication/food.


3. The judge can sign a conservator.

DSM5 will list ways to (no nursing diagnosis)


1. Recognize the diagnosis


2. Treat the diagnosis


3. Variety of the different diagnosis


4. Etiology


Without someone who cannot assess properly then the client will be misdiagnosed.


RN assess + NP, MD assess combine them and look up in the DSM5 to find a diagnosis


Not all clients have the same symptoms in order to meet a specific diagnosis…
