MEXITILENE (MEXITIL)
1- Antiarrhythmic
(tx painful nerve disease)
2- Decreases duration of action potential and effective refractory period in cardiac conduction tissue by altering transport of sodium across myocardial cell membranes/ decreases effects of chemicals on heart = it slows HR
SE- dizziness, nervousness, confusion, dyspnea, blurred vision, tinnitus, chest pain, edema, palpitations, N/V/D, Hepatic Necrosis, tremor, paresthesia, ataxia
3- Prophylaxis, tx of serious ventricular arrhythmia's (VT, PVC) / Do not take if severely weakened or slow HR w/out pacemaker
4- BP, Apical, ECG
Administer w/ food
HOLD if HR <50

ATROPINE

2- Inhibits the action of acetycholine at prostganglionic sites in smooth mm , secretory glands and CNS
SE- dizziness,confusion, dyspnea, blurred vision, dry eyes/mouth,tachycardia,
arrhythmias palpitations, constipation, urinary hesitancy/ retention,
tachypnea, pulmonary edema
3- Used to tx eye irritation, IBS, Ulcers, certain poisonings and SB

4- take 30 mins prior 2 meals
Caution w elderly
Assess VS, I&O, ECG, bowel sounds
ANTIDOTE= Physostigmine
HEPARIN

Assess for S&S bleeding and occult blood,pressure w/ sticks/ bleeding precautions
4- Check PTT & platelets
Antidote = Protamine Sulfate
COUMADIN

Coumadin / Warfarin
1- Coumarins/ Anticoagulant
2- Interferes w/ hepatic synthesis of Vit K dependent clotting factors = prevention of thromboembolic events

3 -tx prophalaxis for thromboembolic disorders : DVT, Pul emboli, A-Fib
Assess for S&S bleeding and occult blood,pressure w/ sticks/ bleeding precautions
4- Check PT ,INR & clotting factors
Antidote = Vit K (phytonadione, AquaMephyton) whole blood w/ severe bleeding
Furosemide

Furosemide / Lasix
1-Loop Diuretic
2- Inhibits re-absorption of Na+ and chloride & increases renal excretion of H2O , Na+, chloride, mag, hydrogen and calcium
SE dizziness, Headache, nervousness, hearing loss, HYPOTENSION, tinnitus, constipation or D/N/V, polyuria, hyperglcemia, dehydration F&E imbalances, metabolic alkolosis,increased BUN, mm cramps
3- tx Edema r/t CHF, hepatic or renal diseae. HTN
MAY cause hypokalemia
4- BP & HR prior to administration
I&O, daily wts, labs, lung sounds
