ferrous sulfate (P.736)
Iron supplement ( antianemic)

Therapeutic Effect: Prevention/ treatment of iron deficiency.

Action: An essential mineral found in hemoglobin, myoglobin, and many enzymes. Enters the bloodstream and is transported to the organs of the liver spleen and bone marrow. Where then it is separated out and becomes part of iron stores.

Common SE: seizures, hypotension, nausea, constipation, dark stools, diarrhea, epigastric pain, skin staining.

RN Considerations: Assess nutritional status, and dietary history to determine cause of anemia. Assess bowel function for constipation or diarrhea. Monitor BP and heart rate frequently following administration. Assess for anaphylaxis.
Iron Dextran ( P.736)
FYI this is same medication as Ferrous sulfate don't know why she did that but double check if you don't believe me
Iron supplement ( antianemic)

Common SE: seizures, hypotension, nausea, constipation, dark stools, diarrhea, epigastric pain, skin staining.

RN Considerations: Assess nutritional status, and dietary history to determine cause of anemia. Assess bowel function for constipation or diarrhea. Monitor BP and heart rate frequently following administration. Assess for anaphylaxis.
Cobalamin (P. 1284)
Vitamin B 12 ( anti-anemic, vitamins)

Therapeutic Effect: Corrects manifestations of pernicious anemia.

Action: Necessary coenzyme for metabolic processes, including fat and carbohydrate metabolism and protein synthesis.

Common SE: Headache,Cyanokit, dizziness, memory impairment, restlessness, hypertension, erythema, rash, red urine.

RN Considerations: assess patient for signs of Vitamin B12 deficiency, ( pallor, neuropathy,psychosis....)
Folic Acid (P. 606)
folate, folvite (anti anemics, vitamins)

Therapeutic Effect: Restoration and maintenance of normal hematopoiesis.

Action: Required for protein synthesis and red blood cell function. Stimulates the production of red blood cells, white blood cells, and platelets. Necessary for normal fetal development.

Common SE: rash, irritability difficulty sleeping, malaise, confusion, fever.

RN Considerations: Assess patient for signs of megaloblastic anemia ( fatigue, weakness, dyspnea) before and periodically throughout therapy.
Cyclosporine (P. 387)
Neoral sandimmune, gengraf (immunosuppressants, antirheumatics)

Therapeutic Effect: Prevention of rejection reactions. Slowed progression of rheumatoid arthritis or psoriasis.

Action: Inhibits normal immune responses, ( cellular and humoral) by inhibiting interleukin-2, a factor necessary for initiation of T cell activity.

Common SE: Seizures, tremors, hypertension, vomiting, nephrotoxicity, hirsutism, gingival hyperplasia, hypersensitivity reactions, infections.

RN Considerations: Monitor serum creatinine level, intake and output ratios, daily weight, and BP, Report significant changes. Assess for symptoms of organ rejections throughout therapy. Monitor for signs and symptoms of hypersensitivity.
Epogen ( P. 512)
Epoetin EPO ( anti-anemics)

Therapeutic Effect: Maintains and may elevate RBC's , decreasing the need for transfusions.

Action: Stimulates erythropoiesis ( production of red blood cells)

Common SE: Seizure, headache, CHF, MI, Stroke, Thrombotic events, hyptertension.

RN Considerations: Monitor BP before and during therapy. Inform HCP if severe hypertension is present or if BP begins to increase. Monitor symptoms of anemia. Monitor dialysis shunts, heparin doses may need to be increased.
Neupogen ( P. 576)
Filgrastim ( Colony- stimulating factors)

Therapeutic Effect: Decreased incidence of infection in patients who are neurtopenic from chemotherapy of other causes. Improved harvest of progenitor cells for bone marrow transplantation.

Action: A glycoprotein, filgrastim binds to and stimulates immature neutrophils to divide and differentiate. Also activates mature neutrophils

Common SE: Excessive leukocytosis, pain, redness, at subcut site, medullary bone pain.

RN Considerations: Monitor vitals before and periodically during therapy. Assess bone pain throughout therapy. Pain throughout therapy. Pain is usually mile to moderate and controllable with non-opioid analgesics, but may require treatment with opioid analgesics, esp in patients receiving receiving high- dose IV therapy.
Oprelvekin (p.953)
Neumega ( colony- stimulating factors) ( prevention of severe thrombocytopenia)

Therapeutic Effect: Increased platelet count

Action: Stimulates production of megakaryocytes and platelets.

Common SE: dizziness, headache, insomnia, nervousness, weakness, conjunctival hemorrhage, cough, dyspnea, pleural effusions. A fib, edema, palpations, syncope, tachycardia, vasodilation, ventricular arrhythmias, anorexia, constipation, diarrhea, dyspepsia, mucositis, nausea, oral moniliasis, vomiting.

RN Considerations: Assess patient for signs of fluid retention during therapy. Fluid retention, is a common side effect that usually resolves within several days following discontinuation of oprelvekin.
