Protonix/ Pantoprazole
Classification
action
precautions
side effect
RN implications
Classification:
- antiulcer agents
- proton-pump inhibitors

Action
- binds to an enzyme in the presence of acidic gastric pH, preventing final transport of hydrogen ions into the gastric lumen

- reduces accumulations of acid

Contraindications:

- hypersensitivity

Side effect:

- headache
-abdominal pain
-diarrhea
- hyperglycemia

RN implication

- Assess patient routinely for epigastric or abdominal pain & for hidden blood in stool, emesis or gastric aspirate.
Plavix/ clopidogrel

Classification
Action
Precautions
Side effect
RN implications
Classification:

- anti platelet agents
- platelet aggregation inhibitors

Action:

- inhibits platelet aggregation by irreversibly inhibiting the binding of ATP to platelet receptors.

Contraindications:

- hypersensitivity
- pathologic bleeding
- lactation.

Side effect:

- depression,
- dizziness

RN implications:

- Monitor patient for signs of thrombotic thrombocytic purpura.

- assess patient for symptoms of stroke, peripheral vascular disease during therapy.
Digoxin/Lanoxin

Name Classification, Action, Precautions, side effect, RN implications?
Classification

- Therapeutic: antiarryhythmics, inotropics

- Pharmacologic: digitalis, glycosides.

Action

- increases force of myocardial contraction.

- prolongs refractory period of the AV node.

Therapeutic: increases cardiac output, slowing of the Heart Rate.

Contraindications

-hypersensitivity, Uncontrolled ventricular, arrhythmias, AV block.

Side effect:

- CNS: fatigue, headache
- EENT: blurred vision
- CV: arrythmias, bradycardia

RN Implications:
- monitor ECG throughout IV administration and 6 hours after each dose. Notify health care professional if bradycardia or new arrhythmias occur.
Metaprolol/ Beloc

Name Classification, Action, contraindications, side effect, Rn implications?
Classification:

- Therapeutic: antianginals, antihypersensitivity

- Pharmacologic: beta blockers

Action:

- blocks stimulation of Beta-adrenergic receptors (myocardial)

- therapeutic: decreases BP, HR

Contraindications:

- Bradycardia or heart block
- cardiogenic shock
- pulmonary edema

Side Effect

- CNS: fatigue, weakness, anxiety, depression

- EENT: blurred vision, stuffy nose

- CV: Bradycardia, CHF, Pulmonary edema, hypotension.

RN implication:

- monitor vital signs and ECG every 5-15 min. during and for several hrs. after parental administration. If HR is less than 40 bpm administer atropine 0.25 - 0.5 mg IV.

- monitor intake and output ratios, and daily weights.
