Acetaminophen


Black box alert:


Potential for severe liver injury.

Class:


Pharmacotherapeupic: Analgesic


Clinical: Non-narcotic analgesic/antipyretic



Uses: Releif mild-moderate pain/fever


IV: Management of moderate to severe pain when combined with opioid analgesia



Contraindications: Hypersensitivity to acetaminophen/severe hepatic impairment or severe liver disease



Cautions: Sensitivity to acetaminophen/severe renal impairment/alcohol dependence/hepatic impairment/active hepatic disease/chronic malnutrition/hypovolemia/G6PD deficiency/limit dose to 4 g a day



Action: Inhibits prostaglandin synthesis in CNS/results in antipyresis/produces analgesic effect



Pharmacokinetics:


PO: Onset less than 60 mins/peak 1-3 hrs/duration 4-6 hrs



Interactions: Alcohol/hepatotoxic medications/warfarin/St.John's wort



Labs: May increase serum ALT/AST/bilirubin/prothrombin levels



Availability (Tylenol): 325mg/500mg/650mg



Administration/handling:


PO: Give without regard to meals/tablets may be crushed/do no crush extened release



Indications:OTC/limit 3000mg a day



Side effects: Rare Hypersensitivity reaction



Adverse effects/toxic reactions:


Early: Anorexia/nausea/diaphoresis/fatigue


Late: Vomiting/RUQ pain/elevated LFTS within 48-72 hours after ingestion



Nursing:


Baseline assessment: Access onset/type/duration of pain/location


Intervention/evaluation: Assess for clinical improvement of pain/releif of pain


Patient/families teaching: Consult physician for children under 2 years/oral use longer 5 days children/or longer than 10 days adults/fever lasting longer than 3 days



Insulin Aspart pen (Novolog)

Class:


Pharmacokinetics: Exogenous insulin


Clinical: Antidiabetic



Use: Diabetes type 1/type 2 to improve glycemic control



Precautions: Hypersensitivity to insulin/use during hypoglycemia



Action: Regulates metabolism of carbohydrates/protein/fat



Therapeutic effect: Controls serum glucose levels



Pharmacokinetics:


Onset 10-20 mins/peak 1-3/duration 3-5



Interactions: Alcohol/beta blockers/glucocorticoids/thiazide diuretics/garlic/ginger/ginseng



Lab values: May decrease serum magnesium/phosphate/potassium



Availability: 100 units/ml vial/ 3ml cartridge/3ml flex pen



Administration: SQ/intravenously infusion/can mix with NPH (aspart 1st)/after 1st use stable room temp for 28 days/administrator 5-10 mins before meals



Side effects: Localized redness/swelling/itching



Adverse effects: Severe hypoglycemia



Nursing:


Baseline: Check BG levels


Interventions: Access for hypoglycemia/cool wet skin/tremors/dizziness/headache/anxiety/tachycardia

Clopidogrel

Nursing
