Primary Hypertension

High blood pressure for which no medical cause can be found; about 90-95 percent of cases

Secondary Hypertension

High blood pressure caused by a condition effecting the kidneys, arteries, heart, or edocrine system; Code for underlying cause first & then for hypertension

Hypertension with Heart Disease

Code separately unless there is documentation states there is a relationship

Hypertension with Chronic Kidney Disease

There is a presumed cause-and-effect relationship; Code Hypertensive CKD, then code for stage of kidney disease & specific type of heart failure (if applicible)

Hypertensive Cerbrovascular Disease

Code for cerebrovascular condition first, then hypertension

Uncontrolled hypertension

May refer to untreated or not responding to current treatment

Myocardial Infarction (MI)

Sudden decrease in coronary artery blood flow resulting in death of heart muscle; heart attack


- If patient presents with symptoms less than 4 weeks post-MI, it is considered acute.


- If patient requires continued care or is symptomatic after 4 weeks, code for appropriate aftercare is used

Old MI

Patient is asymptomatic & does not require any further treatment

Pericardiocentesis

Drawing off collected fluid (via specialized needle) bulit up inside the pericardial sac

Cardiac Tamponade

Compression of the heart caused by fluid collecting in the pericardial sac.

Removing an old implant/pacemaker and inserting a new implant/pacemaker

The removal & insertion will be two separate codes.
