What is osteoarthritis?
Degenerative joint disease which results in degradation of ARTICULAR CARTILAGE and subchrondral bone
What is the difference between primary and secondary OA?
Primary = if caused by age

Secondary = if caused by an identifiable factor
Describe OA in terms of mechanical and biochemical factors?
OA is a MECHANICALLY DRIVEN but BIOCHEMICALLY MEDIATED process
What is the most important cytokine in OA?
IL-1B

- sets off proteinases which cause cartilage destruction
Besides cartilage, what else is damaged in OA?
Bone and synovium
What is seen in terms of chondrocyte activity/numbers in OA?
Increase in chondrocytes (hypertrophy) in order to fix the joint damage
What is one of the main reasons why chondrocyte repair is not all that effective?
Because cartilage is very dense, thus hard to migrate to site of injury
What are fibrillations in OA?
Tiny tears/damage

- Bone loses "smooth" appearance
-Looks like fibers coming off bone
What are osteophytes?

Where are they usually seen?
Bony outgrowths

Usually seen to the side of the joint
What is subchondral sclerosis?
When the bone beneath the cartilage becomes thickened
What is seen in the synovium in OA?
1. Cartilage Fragments

2. Fibroblasts

3. Macrophages
In OA, when do you get pain?
Get pain with use

Stiffness in the morning, but w/in 10 minutes gets better

Bouchards: PIP Joint

Heberdens: DIP joint

DIP - recall: different that RA in which PIP is most commonly affected

1st MCP joint (thumb)
What is the difference between OA and RA in terms of cause of disease?
OA: mechanically driven

RA: immune system attacking joint
What is the difference between OA and RA in terms of age of patients?
OA: Old

RA: any age
What is the difference between OA and RA in terms of joint distribution?
OA: DIPs --> feels "boney"

RA: MCP/PIP and wrist --> feels "squishey"
What is the difference between OA and RA in terms of organs affected?
OA: ONLY joints

How is OA treated?
