Elevate bed and place in high fowlers

Prepare 2 pieces of tape


Set up suction


Put on gloves

Properly measure the tube length to be inserted


Mark tube


Assess nares


Drape chest and hand pt emesis basin and tissue


Lub tip 2-4 inches

Have pt flex head back


Gently insert directing along floor of nose


When pharynx is reached, tuck chin


Sip water and only insert while swallowing


Do not force/rotate the tube if resistant

If gagging and coughing persist, stop and check placement


Comfort and reassure pt


If curled, straight tube and continue


Discontinue the procedure if there is distress

Apply skin barrier to nose


Secure tube loosely to the nose


Aspirate and measure pH


Assess contents


Measure remaining length and secure to gown

Verbalize x ray order


Remove gloves and tape tube to nose


Pin tube gown and place vent at shoulder level


Attach to suction

Remove from suction


Don gloves and fill syringe with 30 mL


Place syringe in tube and unclamp


Do not force solution


Observe for return flow and connect to suction

If unable to irrigate, reposition and try again


If it still doesn't work, consult with primary


Tube may be positioned against gastric mucosa making it difficult to irrigate

Perform oral cares


Remove gloves


Position in more comfortable position


Rinse reusable equipment

Document procedure, tube size, measurements, nare used, suction setting, irrigation, content's characteristics, pH, I&O, and pt tolerance
