Name the Bile Acid Sequestrants (BAS)
Cholestyramine, Colestipol, Colesevelam
Example of Selective cholesterol absorption inhibitor. How does it work?
Ezetimibe, binds on the brush border of the intestinal epithelial cell preventing cholesterol absorption, no effect on vitamin absorption
How does the Bile acid sequestrants work?
They exchange chloride ion for negaively chaged bile acid causes an increase in the number of LDL receptors in the liver
Side Effects of Bile Acid Sequestrants?
Compensatory induction of HMGCoa reductase enhancing cholesterol biosyntehesis partially negating the LDL drop
B. Raises Triglycerides
C. Bloating
D. Might affect absorption of Fat Soluble vitamins and other drugs
What does Nicotinic Acid do in the body?
Decreases VLDL production, raises HDL, and lowers LP(A)
How does Nicotinic Acid do its function?
Inhibits adipose breakdown of fat which is how liver makes VLDL.
Side effects of Nicotinic Acid
Effects:
A. Liver Transaminase activity
B. Glucose levels
C. Lipid Levels
D. Uric Acid (gout)
E. Increase flushing and Pruritis (Itch)
What is Niaspan
Nicotinic acid derivative with less flushing reported
Name examples of Fibric Acid Derivatives?
Gemofibrozil and Fenofibrate (Newer)
What does Gemfibrozil do in the body?
Lowers Triglyceride and raises HDL, Lowers LDL slightly
What are Gemfibrozil side effects?
In hepatic and renal disease should be avoided
when is fenofibrate used?
Hypertriglyceridemia in patients with pancreatitis
How do Statin's work?
They block the HMG-Coa reducatse enzyme blocks its ability to produce cholesterol so upregulates LDL-R to take out cholesterol from body.
Side Effects of Statins?
Increase Transaminase activity (3x stop taking it), Myositis (muscle pain- elevated CPK activity)
If primary problem is elevated LDL what do you use?
Use Statins, if doesn't work use Niacin or Ezetemibe
If primary problem is Low HDL?
Start Fibrate, **Niacin, or statin
What are the lipid-lowering agents?
Statin, Fibrate, Niacin, Bile Acid Sequestrants, Inhibition of Cholesterol Absorption (Ezetimibe)
