define blepharitis/meibomianitis- etiology, s&s,associated disease
1.inflamation of lid margins and oil glannds
2.caused by staph or demodex in general, angular bleph caused by moraxella
3.symptom-itch,red,burn,water, FBS,worse at end of day or
when waking,
4.signs- thick or red margins, crusting, telangieatic vessels, blocked glands and thick secreations
5.common with dry eye
DDX & Tx of blephartitis/meibomianitis
1.DDx- herpes, rosacea, DES, FB, viral/allergic conjunctivitis, hordeolum, ocular pemphigoid, discoid lupus, & carcinoma
2. Tx- good hx, biopsy or culture suspicious lesion, warm compresses QID, lid scrubs, topical bacitracin/erythromycin, short course tobradex, 50mg doxycycline BID, tx associated causes
Cause, S&S, Tx of
Herpes Simplex
1.pain,itch,red,small clusters of vesicles that rupture and crust over...can lead to cornea/conj involvement
2. cool compress, topical antiviral(trifluridine 9X day or vidarabine 5X day) if cornea involved, systemic antivirals (acyclovir 400mg 5X/day 10days, famciclovir 500mg TID 1 week)
Herpes Zoster- definition, signs & symptoms, associated conditions
1.skin eruption, vesicular ulceration & crusting...reactivation of latent virus in CN5..does NOT cross the midline
2.may see fever, HA, malaise, paresthesia, tingling, burning over dermatome, scarring
3.can lead to entropion, ectropian, punctal stenosis, keratitis, madarosis
Herpes Zoster- Tx
1.cool compresses TID
2.erythromycin/bacitracin TID to skin
3.Oral antiviral (800mg acyclovir 5x/day 1 week, 1000mg Valacyclovir or 500mg famciclovir TID 1 week)
4.Capsascin cream or amytriptyline 25mg po TID for pain
Molluscum Contagiosum- define,signs & symptoms, Tx
1.caused by pox virus, in kids, spreads easily
2.dome shapes, waxy papule
3.seen with follicular conjunctivitis, pannus, SPK, self-limiting
4.Tx- curretage, cryotherapy, cautery
Demodicosis- define, signs & symptoms, Tx
1. infection of hair caused by demodex folliculorum
2.seen with blepharitits, crusting at base of lash, may cause hordeolum
3.lid scrubs
Phthiriasis/Pediculosis- define, signs & symptoms, Tx
1.lice
2.itching, burning, white nits/eggs, lice at lashes, prearicular lymphadenopathy, follicles, injections, blepharoconjunctivitis
3.remove lice with forceps, vasoline/topical ointment to suffocate lice, Kwell/Nix shampoo for scalp, discard/wash all linens and clothes, treat std if involved
Leprosy- define, signs & symptoms, Tx
1.mycobacterium leprae
2.eyelash or brow loss, trichiasis, ectropion, lagophthalmos, keratitis, corneal ulcer
3. Dapsone 100mg po QD, Rifampin 600mg po QD, clofazimine 100mg po QD
