Blepharospasm-
define, S&S,DDx,Tx
1.uncontrolled contractions of orbicularis oculi
2.blinking/twitching of eyelid, tough to pry open lids, absent during sleep
3.look for cause (stress, caffiene) and eliminate, Botox to weaken injections, make sure not neurological, antihistamines
4.DDX- reflex blepharospasm, hemifacial spasm, facial myokymia, Tourette's, trigeminal neuralgia, Huntington's
Types of Blepharospasm
1.Essential- related to basal ganglia, gradual onset 50-70 yo
2.Meige's Syndrome- essential blepharospasm with facial grimace
Bell's Palsy-
define, S&S,DDx,Tx
1.acute onset facial paralysis involving CN7, generally unknown cause (possible herpes or inflammatory cause)
2.unilateral paralysis of side of face, HA, numbness, dry eye, FBS, unable to close eye, decrease VA, limited forehead movement
3.tumor of facial nerve, trauma, CNS disease, herpes zoster, congenital nerve palsy
4.pay attention to onset and time of improvement, AT and ointment Q1H, lid taping, imaging if not improved after 4 months, Acyclovir or Prednisone, surgery and botox can be done
Floppy Eyelid Syndrome-
define, S&S,DDx,Tx
1.chronic papillary conjunctivitis,loose lids, spontaneous lid eversion, common in obese men with sleep apnea
2.red eyes, mucous, irritation often in AM, SPK, rubbery lids
3. GPC, inclusion conjunctivitis, SLK, AKC, medicamentosa
4.AT, corna with erythromycin/bacitracin BID 1 week, metal shield, surgical correction, sleep apnea treatment
Trichasis-
define, S&S,DDx,Tx
1.misdirected lashes caused by eyelid problems
2.redness, FBS, watering, SPK, scarring on cornea
3.AT, erythromycin/bacitracin ointment BID, epilation of lashes, electroepilation if recurrent, wedge resection
