Lidocaine

What is the Class of Lidocaine?

Lidocaine is an Antiarrhythmic

Class 1 sodium channel blocker

Lidocaine

What are the actions of Lidocaine?

Lidocaine is an amide-type local anesthetic.

It is used to treat life-threatening ventricular arrhythmias.

Lidocaine depresses depolarization and automaticity in the ventricles.

Reduces the incidence of ventricular fibrillation.

Lidocaine

What are the Indications for Lidocaine?

Lidocain is used in ventricular tachycardia and ventricular fibrillation refractory to Amiodarone.

PVC's = Premature ventricular contractions

Lidocaine

What are the Contraindications for Lidocaine?

Lidocaine is contraindicated in second-degree Mobitz II and third-degree blocks.

Whenever PVCs occur in conjunction with bradycardia, the bradycardia should be treated first.

Lidocaine

What are the precautions for Lidocaine?

Central nervous system depression may occur.

Which includes, Decreased level of consciousness, irritability, confusion, muscle twitching, and eventually seizures.

Lidocaine

What are the side Effects of Lidocaine?

Lidocaine may cause drowsiness, slurred speech, seizures, confusion, hypotension, bradycardia, heart blocks, nausea, and respiratory and cardiac arrest.

Lidocaine

What is the Dosage for Lidocaine?

Initial dose of 1.0-1.5 mg/kg; repeat boluses every 3-5 minutes at half of initial dose, 0.5-0.75 mg/kg. Until max dose of 3.0 mg/kg is reached.

Once the arrhythmia has been suppressed, a lidocaine drip should be initiated at 2-4 mg/min.

Procainamide

What class drug is Procainamide?

Procainamide is an Antiarrhythmic

Procainamide

What are the actions for Procainamide?

Procainamide is used to treat life-threatening ventricular arrhythmias refractory to other arrhythmias.

Procainamide reduces the automaticity of the various pacemaker sites in the heart.

Procainamide slows intraventricular conduction to a much greater degree than does Lidocaine.

Procainamide

Indications for the use of Procainamide?

Procainamide can be used to treat ventricular tachycardia with a pulse and PVCs.

Premature Ventricular Contractions

Procainamide

What are Contraindications for Procainamide use?

Procainamide should not be administered to patients with severe conduction system disturbances, especially second and third degree heart blocks.

Procainamide

What is a Precaution of Procainamide?

Hypotension

Procainamide

What are side effects of Procainamide?

Procainamide may cause drowsiness, seizures, confusion, hypotension, bradycardia, heart blocks, nausea, vomiting, and respiratory and cardiac arrest.

Procainamide

Dosage for Procainamide?

Draw max dose of 17 mg/kg.

Place it into 250 mL bag.

Run @ 20-30 mg/min

This should be discontinued if any of the following occur:

1. Arrhythmia is suppressed

2. Hypotension ensues

3. QRS complex is widened by 50% of original width.

4. A total of 17 mg/kg of medication has been administered.

Amiodarone

What class drug is Amiodarone?

Amiodarone is a Class III Antiarrhythmic

A potassium channel blocker, that affects all channels

Amiodarone

What are the actions of Amiodarone?

Amiodarone is a first line anti-arrhythmic agent given during cardiac arrest to improve the rate of ROSC with refractory ventricular fibrillation and pulseless ventricular tachycardia.

Amiodarone prolongs the action potential duration in all cardiac tissues

Amiodarone

What are the indications for amiodarone?

Amiodarone is used in life-threatening cardiac arrhythmias such as ventricular tachycardia and ventricular fibrillation.

Amiodarone

What are the contraindications for Amiodarone use?

Amiodarone is contraindicated in breastfeeding patients in cardiogenic shock and those with severe sinus node dysfunction resulting in marked sinus bradycardia, second or third degree AV block.

Amiodarone

What are precautions for use with Amiodarone?

Amiodarone should be used with caution in patients that manifest heart failure because heart failure may be worsened by its Administration

Amiodarone

What are side effects of Amiodarone use?

Hypertension, bradycardia.

Amiodarone

What is the dosage for Amiodarone?

W/o pulse: 300 mg IV/IO, repeated at 150 mg every 3-5 minutes

W/ a pulse: 150 mg IV/IO OVER 10 minutes, repeated as necessary.

Magnesium Sulfate

What class drug is magnesium sulfate?

Magnesium sulfate is a antiarrhythmic.

Also an electrolyte and a smooth muscle relaxant.

Magnesium Sulfate

What are the actions of magnesium sulfate?

Magnesium sulfate is a calcium channel blocker and blocks neuromuscular transmission.

It reduces the incidence of ventricular arrhythmias that may follow an acute myocardial infarction.

Used for years in the management of preterm Labor and the hypertensive disorders of pregnancy, such as preEclampsia and Eclampsia.

Magnesium Sulfate

What are the indications for magnesium sulfate?

Magnesium sulfate is used in torsades de pointes, polymorphic ventricular tachycardia.

Also given for severe bronchospasms.

Magnesium Sulfate

What are contraindications for magnesium sulfate use?

Patients who are in Shock, have persistent severe hypertension, who have third degree AV block, known to have hypocalcemia.

Magnesium Sulfate

What precautions for magnesium sulfate use?

Administer slow IV push. Check deep tendon reflexes. Caution in patients with known renal insufficiency. Hypermagnesia can occur. Have calcium salts available as an antidote in case of serious side effects.

Magnesium Sulfate

What are side effects for magnesium sulfate?

Magnesium sulfate can cause flushing, sweating, bradycardia, decreased deep tendon reflexes, drowsiness, respiratory depression, arrhythmias, hypotension, hypothermia, itching, and rash.

Magnesium Sulfate

What is the dosage for magnesium sulfate?

1-2 g of Magnesium Sulfate diluted in 100mL, over 15 minutes as an IV/IO piggyback. Preterm Labor 2 g over 2-5 mins Torsades give 2 g IV push.
