Passenger-Lie
the relationship to the maternal longitudinal axis (spine) to the fetal longitudinal axis (spine).
Lie - (Transverse)-
fetal long axis horizontal-not accomodate vaginal birth. Shoulder is presenting- C-section needed.
Lie- (Parallel or longitudinal)
fetal long axis is parallel to maternal long axis. Cephalic or breech presentation.
Passenger - Attitude
relationship of fetal body parts to one another.
Attitude - Fetal Flexion
chin flexed to chest, extremities flexed into torso. (normal). Abnormal- can cause longer labor/c-section.
Passenger - Presentation
the part of the fetus that is entering the pelvic inlet 1st.
Presentation - What are the diff. parts that can enter inlet first?
back of the head (occiput), chin (mentum), shoulder (scapula), or breech (sacrum).
What are the different kinds of presentation?
brow, face, breech,complete, frank, footling, shoulder.
Breech presentation
butt enters 1st, usually when abnormalities of the uterus and pelvis or preterm fetus.
Fetal Position
relationship of presenting part of the fetus in reference to its directional position as it relates t o 1 of the 4 maternal quadrants.
Fetal position labeling
1. 1st letter- R or L side of maternal pelvis
2. 2nd letter- presenting part of fetus.
3. 3rd letter- Anterior, posterior, or transverse part of pelvis.
station
measurement of fetal descent in cm w/ station 0 being at level of an imaginary line at level of the ischial spines: - stations above and + is below. They use 2 fingers to tell.
engagement
the presenting part occurs when largest diameter of presenting part reaches or passes through pelvic inlet.
Engagement
determined by vaginal examination. Occurs diff. times for primi's and multi's. Multi's might be ballotable.
Why might one do a digital vaginal exam during labor?
To assess for cervical dilation/effacement, fetal station, fetal position-presenting part-lie, and ROM-intact or ruptured?

Descent, flexion, internal rotation, extension, restitution, external rotation, expulsion.
