What are the goals for full metal crown restoration?
1. Protect the remaining tooth structure
2. Restore occlusal fxn
3. Maintain a healthy preiodontium
Ideally the final restoration should have the same dimensions as the original intact tooth
In vet dentistry, full metal crowns are indicated for fractured, non-vital teeth after endodontic therapy, as well as protection of diseased or weakened vital teeth
Covering the occlusal surface with a cast metal restoration distributes occlusal forces and improves the long-term prognosis of endodontically treated teeth
Studies have shown that full crowns exhibit superior retention and resistance when compared to partial crown designs
In this case a medium grit round diamond bur (size 016) was used for margin prep and a medium grit taper diamond bur was used for crown reduction to the level of hte cut margin
The size 016 round bur had a diameter of ~1.6mm
The preparation should cut into the enamel a depth of 0.5-1mm
The round bur will create a chamfer type margin--which is the preferred margin prep for metal crowns
The prep should have a smooth finish
Should be able to see from all angles the tooth (birds eye view) with no undercuts, etc
For the maxillary 4th PM--the palatal axial wall should be reduced to prevent occlusal interference with the opposing mandibular 1st molar tooth
Also for maxillary 4th PM--make sure that any contact b/w the interproximal region of P4-M1 is eliminated to allow for accurate impressions of the margin.
The cusp over the palatal root should be reduced to nearly flat
In order to maximize retention--a pyramid shaped preparation should be avoided!
The final preparation should be smooth and any remaining sharp edges should be rounded or beveled
The impression should:
record the exact dimensions of the preparation, the architecture of the margins, and the relationship of the prepared tooth to the surrounding dentition
The 2-step putty/wash technique using vinyl-poly siloxane (VPS) impression material will provide a detailed impression of the prepartion for fabrication of the metal crown
An alginate impression for the opposing arch can be taken to provide a low detail model used for checking occlusion.
* I do alginate impressions of MX an MN*
The VPS tray is placed in mouth and wiggled before the putty sets to create an enlarged empression and to allow for the light-bodied wash
Light-bodied wash is placed around the tooth and surrounding teeth that was prepped. Make sure to blow in (using air syringe) or digitally manipulate to get the light-body into the sulcus
Keep the impression tray seated with passive pressure until the VPS has set (~5 min). Excessive pressure will cause an inaccurate impression
Inspect the impression for accuracy and make sure the entire margin is recorded with no bubbles or defects
A bite registration is made with medium pink dental wax or a bite registration wash when the pet is extubated
The underlying putty should not be visible through the final impression
Resin based cement is generally advised for animals since it provides a stronger bond than other cements
