What is JIA?
-juvenile idiopathic arthritis
-systemic disease
-articular and extra-articular
-occurs prior to age of 16
how long do JIA symptoms persist
-more then 6 weeks
what are the 3 subtypes of JIA
-pauciarticular
-polyarticular
-systemic
systemic JIA
-10% of cases
-onset can be at any age in childhood
-no gender difference
-joints and organs are involved
systemic JIA signs/symptoms
-fever/chills
-rash on thighs and chest
-fatigue
-joint inflammation
-muscle atrophy
-enlargement of spleen and lymph nodes
-cardiac changes
-weight loss
polyarticular JIA
-40% of cases
-usually >8 at onset
-affects girls 3x as often as boys
-more than 5 joints (typically in feet, ankles, knees, hips)
-often symmetrical
polyarticular signs/symptoms
-insidious onset
-weight loss
-malaise
-low grade fever
-organomegaly
pauciarticular JIA
-50% of cases
-5:1 girls:boys
-early onset > girls (1-5)
-later onset > boys (10-12)
-fewer than 4 joints
-asymmetrical
pauciarticular JIA signs and symptoms
-usually knee
-stiffness, little pain
-sparing of hip and small joints of hand
potential consequence of JIA
-reduced bone mineral density
-increased fracture risk
-reduced activity level
-reduced CP and muscular endurance
-pain/stiffness=reduced ROM
-reduced function of joint receptors (Reduced balance)
potential treatment
-custom semi-rigid FO
-WB activity (can increase/off set BMD loss)
-exercise (to combat deconditioning)
-aquatic activity
assessments for JIA
-fatigue/work of exercise
-children's omni scale
-pain scales
-assess pre-post exercise
