Parenchymal liver disease, elevated transaminases


- hepatitis: viral, autoimmune


- cirrhosis


- drugs: paracetamol toxicity, erythromycin, phenytoin, flucloxacillin, co-amoxiclav, COCP, some antipsychotics

Extrahepatic causes of obstructive jaundice

- choledochplithiasis
- carcinoma of the pancreas (CA19-9, ampulla (faecal occult blood), bile duct (cholangiocarcinoma)
- chronic pancreatitis
- strictures of the bile duct
- parasitic infection

Intrahepatic causes of obstructive jaundice

- PBC


- PSC


- alcohol


- drugs


- liver mets: hepatomegaly or irregular hepatic masses


- CF


- pregnancy


- Mirrezi syndrome: impaction of a gallstone in the neck of gallbladder

Clinical features of obstructive jaundice

- pale stools


- dark urine: urine bilirubin


- pruritis: usually associated with malignancy
