Examples of Isotonic Solutions

(What is Isotonic to plasma?)
-Lactated Ringers
-0.9% NaCl (Normal Saline)
-D5% in H20 (until glucose is metabolized then it becomes hypotonic)
-Sodium Lactate Solution

Average Serum Osmolality is 290 mOsm/Kg. Isotonic fluids are 240-340.
When are Isotonic Solutions used?
Hypovolemia (blood chemistry ok).
Burns, fluid loss, diarrhea, etc.

-0.45% NaCl
-0.33% NaCl
-D5% in H20 AFTER it becomes metabolized!
When are Hypotonic Solutions used?
Dehydration where electrolytes are still high and you want to supply free water and dilute electrolytes.
Examples of Hypertonic Solutions
-3% NaCl (used for severe hyponatremia)
-5% NaCl (dangerous)
-5% Sodium Bicarb (used to correct severe metabolic acidosis)
When are Hypertonic Solutions used?
Patients who need electrolytes either for deficit or to regain acid base balance.
