Elements of history in immediate newborn care
review maternal O hx, review family hx, review labor hx--gestational diabetes, other deliveries, etc.
Timeframe for APGAR score
1 and 5 minutes
Steps of neonatal resuscitation
position, suction, dry, warm, and stimulate; oxygen; effective ventilation w/ bag valve mask or ET intubation; chest compressions; meds (memorize algorithm)
The majority of kids only need this intervention 9routine care)
Provide warmth, clear airway, and dry
Neonatal resuscitation is done in how many second intervals?
30 second intervals
What are the elements of meconium aspiration syndrome (MAS)?
respiratory distress, pneumonintis, and pneumothorax
What percentage of deliveries is meconium present in? What percentage of those develop MAS?
10%; 15%---> so if you did nothing on delivery, there would be a 1.5% of babies developing MAS
What is the first oxygen delivery method you use in neonatal resuscitation?
100% blow by air (just placed close to head)
Why do we suction the baby's airway prior to the first breath?
to prevent Meconium Aspiration Syndrome
At the point at which a child has received O2, is breathing, has good color, and a HR over 100, what is the next step?
supportive care- warmth, keeping airway clear, and rying baby
At the point at which a child has received O2, but the HR is still below 100 or the baby is apneic, what do you do?
provide positive pressure ventilation
After positive pressure ventilation, if the baby's HR is less than 60, what do you do?
chest compressions
After positive pressure ventilation, if the baby's HR is between 60 and 100, what do you do?
continue Positive Pressure Ventilation until HR is over 100 and then monitor
If you have done chest compressions for over a minute with no improvement, what drug can be admnistered to a neonate?
epinephrine
How much care has been provided (duration) by the time epinephrine would be given?
about 2 1/2 minutes
When is endotracheal intubation considered in neonatal resuscitation?
it can be considered at several steps (however, on the test, choose the next step, not the coverall answer).
Why do we suction the baby's airway prior to the first breath?
clear of meconium?, breathing or crying?, good muscle tone?, color pink? , and term gestation?
Why must care be taken to prevent hyperextension or underextension of the neck in resuscitation?
either will decrease air entry
How is "blow by" oxygen administered?
10 LPM, less than 1/2 from face
How is bag valve mask oxygen administered to neonate?
in the sniffing position, rate of 40-60 BPM
When is intubation urgent?
when there is resistance to BVM
What is meconium?
baby poo
Rate for chest compressions in neonate
120 per minute
Ventilation to compression ratio in necnates
1:03
Two techniques for chest compression
2 fingers, 2 thumbs (preferred)
What is the indication for IV fluid bolus in neonates?
indicated for hypotension (10 cc/kg NS or LR)
How is epinephrine administered in resuscitation?
IV or ET
What is the use of Narcan in neonatal resuscitation?
maternal narcotic use within 4 hours
How much NS or LR is given to a neonate?
10 cc/kg NS or LR
