ACTH Stimulation Test


(Short Synacthen test)

Tests: functioning of adrenal gland stress response by measuring adrenal response to ACTH.


Procedure: small amount of synthetic ACTH is administered and amount of cortisol/aldosterone adrenals produce is measured.


Normal: Cortisol level should double within 60m


Primary Insufficiency/Addisons: Low baseline cortisol, does not rise >25%


Secondary Insufficiency: Dramatic increase in cortisol from baseline

Albumin

Indication: malnutrition, oedema


Decreased: malnutrition, nephrotic syndrome, cystic fibrosis, multiple myeloma, hodgkin's disease, leukaemia, protein losing enteropathy, IBD (chron's/coeliac).

Aldosterone Serum

Function: steroid hormone produced by adrenal cortex. Regulates BP, secretion of K, conservation of Na, retention of water.


Increased: Primary hyperaldosteronism (from adrenal gland), Secondary (overactive RAAs). Conn's syndrome (aldosterone producing adenoma)


Decreased: Hypoaldosteronism
