What is a fully human monoclonal antibody?
-umab, it is a human antibody.
What is a humanized monoclonal antibody?
-umab, a murine antibody that is completely replaced with human regions except for hypervariable regions (CDRs, 3 on each arm) that remain murine.
What is a chimeric monoclonal antibody?
-ximab, murine antibody with human constant regions (Fc portions).
What is a murine antiobdy?
-omab, antibody from a mouse.
How do you create a monoclonal antibody? (1)
Transgenic mice; replacement of Ig genes in a mouse with human Ig genes. Immunize the mouse and then produce mAbs.
How do you create a monoclonal antibody? (2)
Phage libraries; insertions of human Ig gene into a bacteriophage.
How do you create a monoclonal antibody? (3)
Hu-SCID mice. Introduction of human cells into a severely immunocompromised mouse meaning it won't be recognised as foreign and can produce Ig.

1) Infliximab; indicated for RA, Chrohn's and ankylosing spondylitis.

2) Certolizumab; PEGylated Fab fragment of a humanized monoclonal antibody. PEGylation increased its circulatin half life.

Ustekinumab; abrogates NK, Th1 and Th17 mediated release of pro-inflammatory cytokines.
Name an anti-BLyS (B cell activating factor).
Belizumab; BLyS is the anti-dsDNA found in SLE.
Name an anti-IL-6-R mAb.
Tocilizumab; appropriate in severe RA.
Name and anti-CD-20 mAb.
Rituximab; indicated for RA, CLL and non-Hodgkin lymphoma. Binds CD20 and also activates complement (therefore cross-linking CD-20 +ve cells and targetting them for apoptosis.

Natalizumab; indicated for Crohn's and MS.

In Crohn's integrin mediated leukocyte diapedesis in inflamed areas of GI tract is the main cause of pathology. Natalizumab stops this.
