what is the moa of cyclosporine?
binds w/ cyclophilin --> inhibits calcineurin --> inhibits activation of Tcell transcription factor --> decrease IL-2, IL-3, INF-y

inhibits calcineurin -->IL-2, IL-3, INF-y
when is cyclosporine used?
psoriasis, atopic dermatitis, rheumatoid arthritis, keratoconjunctivitis sicca, ulcerative colitis
how is cyclosporine administered?
topical and systemically
what are the adverse effects of cyclosporin?
gum hyperplasia, numbness, tingling, nehprotoxicity, hepatotoxicity --> cholelithiasia, HTN, opportunistic infections, hyperglycemia
what is an alternate to cyclosporin a?
cyclosporin G and has less nephrotoxicity
when is azathioprine used?
organ transplant, autoimmune disease, RA, crohns, ulcerative colitis
what is the moa of azathioprine?
inhibits purine synthesis esp in leukocytes
what are the adverse effects of azathioprine?
nausea, rash, GI mucositis, marrow suppression
what genetic polymorphism predisposes to adverse effects from azathioprine?
polymorphisms in TPMT can lead to excessive drug toxicity
when is azathioprine use cautioned?
when used w/ allopurinol
what other additional risks are associated w/ azathioprine?
is a carcinogen and can increase risk of non-Hodgkins lymphoma, squamous cell carcinoma of skin, hepatobiliary carcinoma, mesenchymal tumor
