What is the diagnostic criteria for hepatorenal syndrome?
- Oxygenation defect - Partial pressure of oxygen <80mmHg or alveolar-arterial oxygen gradient >15mmHg
- Pulmonary vascular dilatation (enhanced echocardiography i.e microbubble opacification of the left heart chambers three to six cycles after right atrial passage)
Liver disease - portal hypertension with or without cirrhosis

What is the unique pathological feature of hepatopulmonary syndrome?
Gross dilatation of the pulmonary precapillary and capillary vessels (to 15-100 microm in diameter when the patient is at rest)

The defect in oxygenation is due to a ventilation-perfusion mismatch characterised by increased blood flow while alveolar ventilation is uniformly preserved

Normal range of the capillary diameter, <8 to 15 microm
Microbubbles >10microm in diameter
What is the treatment for hepatopulmonary syndrome?
Liver transplant is the only successful treatment
