MOA
Inhibits normal blood clotting by enhancing antithrombin III, blocking conversion of prothrombin to thrombin and fibrinogen to fibrin

Indications
1. Prevention of blood clot formation
2. Maintains patency of IV venous access devices

Contraindications
1. Patients on anticoagulants or thrombolytics
2. active bleeding / bleeding tendencies

Side Effects
Cardiovascular: minor to severe bleeding

Precautions
1. Saline/Heparin locks should only be utilized in patients with stable vital signs and no need for volume replacement
2. Heparin may cause local irritation
3. Examine regularly for signs of internal bleeding
4. Do not administer IM injections before or during administration

Interactions
Flush venous access device before and after administration as Heparin may be incompatible with other medications

Routes / Dosages
IV

Adult: 5,000 U (units) IV, then 20,000-40,000 U IV over 24 hours
Pediatric: 50 U/kg IV, then 25 U/kg /hour over 24 hours
