Ferrous Sulfate: PO
Ferrous Gluconate: PO
Iron Dextran: IV
Iron Sodium Gluconate Complex: IV
Uses: Iron deficiency anemia
Pharm: Fe2+ absorbed in duodenum & jejunum, txpted via transferrin, stored as ferritin, rarely eliminated.
Effects: N/V/D, necrotizing gastroenteritis, hemochromatosis, metabolic acidosis, shock, coma
(therapy with deferoxamine)
Deferoxamine: IM, IV
Uses: iron poisoning (iron chelator)
MOA: binds & inactivates Fe molecule
Effects: Neurotoxicity, ARDS, hypotension, allergic reactions
Folic Acid: PO
Uses: Megaloblastic anemia due to folic acid deficiency, prevents NTD during pregnancy, For patients: dialysis, liver dz, phenytoin, MTX
Pharm: absorbed by proximal jejunum, + CSF, stored in liver
FYI: converted to THF w/ vit. B12, needed for purine & DNA synthesis
Leucovorin (Folinic acid): PO
Uses: MTX, TMP & pyrimethamine toxicities, increases Fluorouracil efficacy
Pharm: absorbed in small intestine
FYI: THF cofactor, repletes folate pool
Cobalamin (vit. B12)
Uses: Megaloblastic anemia due to impaired B12 absorption
Pharm: combines w/ intrinsic factor in distal ileum
Effects: hypokalemia, thrombocytosis
FYI: required in enzymatic rxns: N5 CH3-THF + homocysteine --> methionine + THF, CH3-malonyl CoA --> succinyl CoA, At risk for anemia: vegans, malabsorption syndromes, IBD, pernicious anemia
Erythropoietin
Uses: anemias of chronic renal failure
Pharm: stimulation of erythroid proliferation & differentiation
Effects: hypertension & thrombotic complications
FYI: activates JAK/STAT receptors
