Depression and cortisol
People with depression have HIGH cortisol levels

1/3 or depressed people have blunted TSH. In these people treat the thyroid and if the symptoms go away it wasn't depression
What are the primary NT's of depression?
Low levels of serotonin and NE. NOTE: these NT's affect each other because where 5HT goes so will NE (bipolar being the exception)

High NE and low serotonin (these normally follow each other, this is the exception)

Majority will have a major depressive episode right before or after manic episode

What is the key to mood/affect in bipolar patient?
LABILE
Why do manic and depressed pts get constipated? have poor hygiene?
Constipation: Body is slowed or the person is dehydrated, not paying attention to the body
Hygiene: depressed can't manage it, manic get too distracted
What percentage responds to ECT?
85%, improve initially but not a long term cure, reset the brain for medicine to work
What pt population is ECT less effective for?
Older pts with dementia
Who should not receive ECT?
Cardiac pts (b/c ECT has a low incidence of cardiac effects)
