Glyceral Trinitrate

- relaxes smooth muscle


- potent vasodilator (increases venous pooling and decreases blood pressure)


- decreases pre-load


- decreases myocardial oxygen demand


- pain may dissipate


- should be administered by aspirin in most cases


Indications
- autonomic dysreflexia with BP > 160mmHg
- irukanje sting wuith BP > 160mmHg
- suspected AMI/ischaemia
- acute cardiogenic pulmonary oedema

Contraindications

- BP<100mmHg systolic


- concurrent use of viagra or other erectile dysfunction within 24 hours


- HR <50 or >150


- CVA


- Head trauma


- known severe adverse reaction

Precautions

- suspected inferior AMI


- cerebral vascular disease


- risk of hypotension/syncope


- intoxication (effects are enhanced)


- erectile dysfunction medication (in previous 4 days)


Side effects

- dizziness


- hypotension


- syncope


- reflex tachycardia


- vascular headaches

Dosage

400mcg sublingual spray, repeat every 5 minutes. No max. dosage


- AMI: repeat while BP remains > 100mmHg


- CPO: BP > 100mmHg


- Autonomic dysreflexia: until BP normalises

Effects

- Onset: 30sec - 2min


- Peak: 4min


- Duration: 20-30min


- Half-life: 5.5min
