KIDS (at the fair)
What toxin makes EHEC (0157) so very bad?
Shiga-like toxin aka verotoxin
What is the major sequelae due to Shiga-like toxin?
Hemolytic Uremic Syndrome (HUS)

HUS causes acute renal failure in KIDS!
How would you diagnose HUS?
1. MAHA (MicroAngiopathic Hemolytic Anemia): damage to glomerular microvasculature
2. Thrombocytopenic purpura
3. Acute renal failure
4. Young kids
5. Caused by 0157 EHEC's shiga-like toxin
6. primary target: renal microvasculature
Should you give antibiotics to patients with EHEC infection?
NO!!!!!
Abx will make MORE Shiga-like toxin!
Does C. dif use spores?
Yes
What are the two toxins associated with C. dif and what do they do?
Toxin A: screws up tight junctions in enterocytes so that crap leaks out of cell (and B can get in too?); stimulates release of Substance P, Neurotensin, Calcitonin gene-related peptide. Now you get secretagogues activated-->Cl- secretion and H2O loss

Toxin B: enters from basolateral side

INDUCES APOPTOSIS
What is the defining clinical feature of C. dif?
Pseudomembranous colitis

plaques!
Is C. dif part of normal flora?
Yes
What kind of food might have C. dif in it?
poultry
How are toxins A and B different from cholera/listeria toxin?
Toxins A and B cause inflammation and fluid secretion; CT/LT do not.
What are the 5 Ps of EPEC?
1. Pathogenic
2. Pedestal (caused by attaching and effacing lesions; decreases absorptive surface area)
3. Persistent diarrhea
4. Pili
5. Peds
What is the colonization pattern that you'd see with EAEC?
Bricks!
In what kind of population will you most likely see EAEC?
1. HIV/AIDS
2. kids in poor countries
