BADL vs IADL

Basic - everyone needs to do


Instrumental - don't need to do on a daily basis

Nagi Disability Scheme

-based on a medical diagnosis


Active Pathology - med diagnosis


Impairments - ROM, strength, balance measures


Fxnl Disability - certain tasks are impaired


Disability - impairs ability to fill certain roles

ICF Conceptual Model

-like the Nagi but takes environmental and personal factors into account

Functional Independence Measure (FIM)

-determine the degree of disability using direct observation of patient performing tasks


-uses a 7-point scale


-use on adult population

What does FIM predict?

-burden of care

Problems with FIM

-it only assesses BADL


-doesn't measure the impact of disability on the individual

Barthel Index

-a 10-item ADL that assesses the ability of the individual to do self care


-self report or direct observation


-takes 25 mins


-use on elderly and chronically ill

Barthel Index Interpretation

100 - patient can do self care


60 - probably needs assisted independence


<40 - low probability of assisted independence

Barthel Index Problems

-does not measure IADL

Stroke Impact Scale (SIS)

-59 items with 8 domains


-takes 20-30 mins


-each item rated on a 5 point scale indicating difficulty the patient has experienced with it

Physical Performance Test (PPT)

-assess a Pt's level of fxn as indicated by taaks that stimulate ADLs


-scored via timed direct observation


-takes about 10 minutes


-use with elderly and parkinsons

Pain Disability Index

-a self report measuring the amount of pain-related disability


-takes less than 5 minutes


-use on adult with chronic pain

Patient-Specific Functional Scale (PSFS)

-self report that is used mainly in orthopedics


-they ask the Pt to name 5 activities they have difficulty with and give them a score from 0=unable to perform, to 10 =able to perform

Problems and Benefits of PSFS

P - can't compare patients


B - patient specific and can guide treatment

Global Rating of Change (GRC)

-asks Pt to recall their health status at a previous time and calculate the difference between then and now

Concerns with GRC

-can't perform on people with cognitive deficits


-it's hard for Pt's to remember prior health status
