If wx is below landing MIN, T/O ALT must
be within:
400 NM
When is a destination alternate required?
• If DEST is not a R, A, F, or P, list an ALT
• If DEST & ALT wx marginal, list 2nd ALT
- If wx is below:
1-2-3 Rule
1-1-3 Rule
1-1-2 Rule
• Crosswind An alternate will be designated whenever crosswind performance guidelines may prevent a safe landing at the destination.
• Unlighted Airports A flight scheduled to arrive at an unlighted airport less than 30 minutes before sunset must have a lighted alternate.

FOM 4.20.1 2 AUG 13
1-2-3 Rule
ALT required if forecast:
• +/- 1 hour of ETA is < 2000’ or 3SM

FOM 4.20.3 2 AUG 13
1-1-3 Rule
- CAT I acft and rwy avail and
legal with 200’ DA & RVR of 2000’ or lower
- Within 48 states
- +/-1 hour of ETA is < 1000’ or 3SM (1-1-3)
-NOT AUTH IF TS (PROB or TEMP) +/-1 hour of ETA

FOM 4.20.3 2 AUG 13
1-1-2 Rule
CAT II/III acft and rwy avail &
legal
- Within 48 states
- +/-1 hour of ETA is < 1000’ or 2SM (1-1-2)
- Not auth if TS (PROB or TEMP) +/-1 hour of ETA

FOM 4.20.3 2 AUG 13
Maximum holding speeds
0 - 6000 ft = 200 kts
6001 - 14000 ft = 230 kts
above 14000 ft = 265 kts

FOM 6.70.2 12 APR 13
Stabilized approach gate at 1500 feet AGL/RA or the FAF, whichever occurs later:
– Landing gear down.
– Airspeed no greater than 180 KIAS.

FOM 6.80.1 12 APR 13
Stabilized approach gate by 1000 feet AGL/RA:
– Final landing configuration
– Landing Checklist/Final Descent Checklist complete
– Airspeed within +15 and -5 knots of target airspeed
– On lateral profile
– On vertical profile, or correcting with bracketing maneuvers not to exceed:
♦ ±300 FPM
♦ ±1°
♦ Sustained vertical speed greater than 1200 FPM

FOM 6.80.2 12 APR 13
Stabilized approach gate report at 1000 feet AGL/RA:
• PM will announce deviations in configuration, speed, or descent rate from the plan
(e.g., “Final flaps to go,” “Airspeed,” or “Descent rate”).

FOM 6.80.2 12 APR 13
Stabilized approach gate reject at or below 500 feet AGL/RA:
• Except for momentary airspeed and descent rate deviations, the PM will direct a go-around if any of the planned parameters are not met by announcing “Go-around” followed by the out-of-tolerance parameter (e.g., “Flaps,” “Airspeed,”
or “Descent rate”).

FOM 6.80.2 12 APR 13
Other than the conditions of exemption 3585, when is a second alternate required?
When the weather forecasts for both the destination and first alternate airport are marginal (i.e., at the legal minimums to be listed on the Dispatch Release), at least one additional alternate airport must be designated.

FOM 4.20.3 2 AUG 13
When are enroute alternates required and what are the weather requirements for enroute alternates?
- Enroute alternates are required when an engine out driftdown alternate is required (Method II Dispatch
- Weather requirements for enroute alternates are identical to those of destination alternates.

FOM 4.20.3 2 AUG 13
What types of airports are authorized for use as alternates?
- Regular.
- Provisional.
- Refueling.
- Alternate.

FOM 4.20.3 2 AUG 13
Alternate weather minimums
• For airports with one operational approach facility:
– Add 400 feet to the authorized MDA(H) or DA(H).
– Add 1 mile/1600 meters to the minimum visibility.
• For airports with two operational approach facilities to different runways, use
the lowest values obtained from each of the following:
– Add 200 feet to the higher authorized MDA(H) or DA(H) specified for the two
approaches, or add 400 feet to the lower authorized MDA(H) or DA(H) specified
for the two approaches.
– Add 1 mile/1600 meters to the lower visibility minimum specified for the two
approaches, or add 1/2 mile/800 meters to the higher visibility minimum
specified for the two approaches.

FOM 4.20.7 20 DEC 13
Domestic fuel reserve requirement
• To fly to the airport to which it is dispatched;
• Thereafter, to fly to and land at the most distant alternate airport (where required) for the airport to which dispatched;
• Thereafter, to fly for 45 minutes at normal cruising fuel consumption.

FOM 4.30.1 12 APR 13
Flag 10% fuel reserve requirement
• Fly to and land at the airport to which it was released.
• Fly for a period of time equal to 10% of the planned flight time between the
departure and destination airports.
• After that, fly to and land at the most distant alternate listed in the Dispatch Release.
• After that, fly for 30 minutes at holding speed at 1500 feet above the alternate (or above the destination if no alternate is listed in the Dispatch Release).

FOM 4.30.2 12 APR 13
Flag two hour reserve rule
When dispatching to a destination approved with no alternate, there must be sufficient
fuel on board to fly to and land at the destination, and after that to fly for at least 2
hours at normal cruising fuel consumption (FAR 121.645[c]).
Note: This requirement does not apply when no alternate is required under the Six-Hour Rule.

FOM 4.30.2 12 APR 13
Flag 5% fuel reserve requirement
• Forecast destination weather must be at or above minimums at ETA.
• At least once every 90 minutes, a position report with fuel on board must be sent to Dispatch.
This authorization shall not be used in combination with Redispatch operations.

FOM 4.30.2 12 APR 13
Required Dispatch reports
• If actual ETA will exceed planned by more than 15 minutes, or there is an accumulated deviation of +/- 5 minutes from a previously reported estimate
• If cruise altitude varies by 4000 feet or more from the flight plan
• If a lateral deviation from the planned route exceeds 100 miles
• Any enroute failure of a flight deck fuel quantity indicator
• If it becomes apparent that all contingency fuel will be burned (i.e., ACF, EXTRA, and HOLD) and therefore part of the reserve fuel will be burned, coordinate a plan of action with Dispatch and provide the details of what occurred (e.g., holding at destination)
• A flight encounters weather significantly different than forecast, or any time moderate or greater turbulence is encountered
