0.9% Normal Saline (has 0.9% sodium)

Universal Fluid...commonly used...usually first line
pH 5.5

0.45% N. Saline

Use if patient in HHS (Hyperosmolar Hyperglycaemic State) and their Na > 150

5% Dextrose

Fluid that is closest to water
Give in Hypernatraemia (NA > 150)

1.8% N. Saline

ONLY given if in HDU (High Dependency Unit) and if patient is profoundly hyponatraemic (Na < 105) and if they are fitting

10% dextrose

- Give in people who are having VRII (variable rate insulin infusion)...usually if patient has DKA

- People with hypo's get it (cause of hypo may be diabetes, sepsis etc.)

20% Dextrose

- Profound Hypoglycaemia

- Hypokalaemia (as you give insulin as part of treatment you need to give dextrose to prevent dangerous hypo's)

50% Dextrose

- Hypoglycaemic coma

Hartmann's Solution

- This has 5mmol of K+ in it
- Used as a maintenance fluid...so if patients have been on N Saline for > 24 hours use this
- DO NOT give to someone in AKI due to potassium content

1.26% Sodium Bicarbonate

If patient has Hyperkalaemia with bicarb < 22 give this if other treatment isn't bringing potassium down.

DO NOT give in children (can cause encephalopathy)

As in kidneys sodium is exchanged for hydrogen ions or potassium...knocking out hydrogen ion content with the sodium bicarb means potassium is what is exchanged for sodium

8.4% Sodium Bicarbonate

- Used in a cardiac arrest situation whilst doing CPR. Usually in ICU

HAS 4.5% (albumin solution)

- Use in chronic liver disease (as hypoalbuminaemia occurs)


- Use after ascitic tap (as otherwise the space will fill up with fluid again)

HAS 20% (albumin solution)

- Use in extreme liver disease


- Massive ascited


- Fluid overloaded patient

FFP (Fresh frozen plasma)

- Is a part of the trauma bundle

- Part of massive transfusion bundle


- Give if pt who is on warfarin has INR > 8 and is bleeding

Cryoprecipitate

Give in patient with DIC (disseminated intravascular coagulation)

Platelets

Give if platelets < 100 and active bleeding (with specialist advice)




If pt not bleeding give platelets if < 10
