Hodgkin's vs Non-Hodgkins...

nodes?
symptoms?
age incidence?
prognosis?
other?
Nodes:
-Hodgkins - localized single group, extranodal rare, contiguous spread
-Non-Hodgkins - multiple, peripheral nodes, extranodal common, non-contiguous spread

Symptoms:
-Hodgkin's - consitutional B signs - fever, night sweat, weight loss
-Non-Hodgkins - fewer constituational symptoms

Age:
-Hodgkins - bimodal distribution, young and old
-Non-Hodgkins - peak incidence for certain subtypes at 20-40 years

Prognosis:
-Hodgkins - stage strongest predictor (contiguous spread); better w/increased lymphocytes, fewer RS cells
-Non-Hodgkins - ?

Other:
-Hodgkins - mediastinal lymphadenopathy, Reed Sternberg cells, 50% EBV associated
-Non-Hodgkins - may be associated w/HIV or immunosuppression
CD30+ and CD15+ indicates?
Reed Sternberg cells
B-cell origin
binucleate or bilobed "owl eyes"

"necessary but not sufficient for dx of Hodgkin's dz"
localized, single group of nodes with...

1.) collagen banding & lacunar cells young woman?
2.) numerous binucleate cells?
3.) man under 35?
4.) older man?
1.) Nodular sclerosing (65-75%)
-excellent prognosis
-many lymphocytes, few RS

2.) Mixed Cellularity (25%)
-intermediate prognosis
-many lymphocytes, many reed sternberg cells

3.) Lymphocyte predominant (6%)
-excellent prognosis
-many lymphocytes, few reed sternbergs

4.) Lymphocyte depleted (rare)
-poor prognosis (older males w/disseminated dz)
-RS high relative to lymphocytes, but few lymphocytes
lacunar cells in lymphoma?
Nodular sclerosing Hodgkin's
how to distinguish leukemoid reaction from leukemia?
increased LAP in leukemoid reaction

neutrophils out to "LAP" up the infection
in leukemia they're partying, no intention of fighting infection
Translocations, what gene, what dz?

t(11,14)
t(14,18)
t(8,14)
-c-myc moves to IgH
-Burkitt's lymphoma

t(11,14)
-cyclin D moves to IgH
-mantle cell lymphoma

t(14,18)
-bcl2 moves to IgH
-follicular lymphoma
lymph node biopsy w/sheets of lymphocytes interspersed w/macrophages?
Burkitt's lymphoma
"starry sky"

t(8,14) - c-myc

adolescents or young adults

EBV associated
Jaw - endemic form in Africa
Pelvis/abdomen - sporadic form
MC adult NHL?
diffuse large B-cell lymphoma
(many things progress to this)

usually in older adults
20% in children

May be mature T cell in origin (20%)
lymphoma w/CD5+?
Mantle cell lymphoma

t(11,14) - cyclin D

older males
NHL difficult to cure w/indolent course?
Follicular lymphoma

t(14,18) - BCL2
(inhibits apoptosis)

adults
lymphoma presenting w/cutaneous lesion?
Adult T-cell lymphoma

HTLV-1 causes

presenting w/cutaneous lesions
affects Japan, West Africa, Caribbean

aggressive

adults
lymphoma presenting w/cutaneous patches / nodules?
Mycosis fungoides
Sezary syndrome

indolent CD4+

adults
patient w/EBV is found to have mediastinal lymphadenopathy?
Hodgkin's lymphoma
