Sodium Bicarbonate
Class: Alkalinizing agent

Pharm: raises PH and binds tricyclics to serum protiens by alkalinizing

Indications: Tricyclic antidepressant overdose; cardiac arrest with long downtime; crush injury pretreatment.

Contra: Hypoxic lactic acidosis

Side FX: alkalosis, worsens cellular acidosis in hypoperfusion states

Dosage: 1.0 mEq/kg of 8.4% IVP
Crush injury: 1.0 mEq/kg IV and/or 0.25 mEq/kg/hr infusion
Verapamil
Class: Calcium Channel Blocker

Pharm: relaxation of vascular smooth muscle, causing vasodilation. Slows conduction through AV node decreasing HR, decreases rapid ventricular response with atrial tach.

Indications: PSVT refractory to Adenosine; Control of rapid V response with A-Fib and A-flutter

Contra: Hypotension, Cardio shock; V-tach

Side FX: Hypotension, N/V, headache, dizziness

Dosage: 2.5--5mg over 2-3 min IV; may repeat in 15-30 min with 5--10mg over 2-3 min. MAX dose 20mg
Magnesium Sulfate
Class: Electrolyte, Antiarrhythmic

Pharm: depressant of smooth, cardiac, and skeletal muscle by decreasing acetylcholine. CNS Depressant

Indications: Recurrant/refractory V-Fib/pulseless V-Tach; Torsades de Pointes; Convulsions/siezures from toxemia due to pregnancy.

Contra: Heart blocks, hypotension

Side FX: respitory depression, bradycardia

Dosage: V-Fib/PL V-tach after lidocaine 1--2g IVP;
Torsades 1--2g in 100mL over 5-60 minutes drip;
Pregnancy 2--4g IVP over 3 min followed by 1--2g/hr drip;
Post infarction/Hyperkalemia 1--2g in 100mL over 60 minutes
Amiodarone
Class: Antidysrhythmic

Pharm: Prolongs duration of the action potential and effective refractory period. Decreases AV conduction velocity and sinus node function.

Indications: VF and PLVT refractory to defib; torsades de pointes; rate control in A-Fib or A-flutter when all else fails.

Contra: Hypotension, hypersensitivity

Side FX: vasodilation, hypotesnion

Dosage: VF/PLVT 300mg IVP, after 3-5 min repeat 150mg IVP
Wide Complex Tach 150mg over 10 minutes, may repeat every 10 min as needed.
Main infusion 1mg/min for 6 hours
