Alteplase (tPA)
Converts Plasminogen to Plasmin.
Needs FIBRIN to work
Indications: MI, CVA, Pulmonary Embolism.
Used in ischemic strokes, not hemorrhagic strokes
Adverse effects: Hemorrhage
Streptokinase
Causes degradation of fibrinogen, fibrin, factors V and VII
Indications: MI, DVT, PE, CVA and other arterial thromboses
Adverse Effects: Hemorrhage and fibrinogen, factor V and VII depletion
Urokinase
Approved for Pulmonary emboli but also used for DVT, MI, arterial thromboembolisms
Low antigenicity, highly specific for fibrin
made in kidney
can cause severe hemorrhage

CONTRAINDICATIONS:
Surgery within 10 days

GI Bleeding in last 3 months
